Job summary
We are looking for a confident, enthusiastic and patient-focused Clinical Pharmacist to join our friendly and supportive clinical team on a full-time basis. This is an exciting opportunity for a pharmacist with GP practice experience who is passionate about delivering high-quality, safe and effective patient care within primary care.

As a Clinical Pharmacist, you will work as an integral part of the multidisciplinary team, using your clinical expertise to optimise medicines, improve patient outcomes and promote the safe and effective use of medicines. You will undertake structured medication reviews, support patients with long-term conditions, provide clinical advice to patients and colleagues, and contribute to medicines optimisation and quality improvement initiatives.

Main duties of the job

• Undertake structured medication reviews and medicines optimisation to improve patient outcomes safety and adherence.
• Provide clinical advice to GPs, nurses and the wider multidisciplinary team on prescribing and medicines-related issues.
• Support the management of patients with long-term conditions, including medication review, monitoring and follow-up.
• Identify and manage medication-related risks, including interactions, adverse effects, contraindications and inappropriate prescribing.
• Support medicines reconciliation following hospital discharge and changes in care.
• Provide patients with clear advice and counselling on their medicines, promoting adherence and shared decision-making.
• Support safe, effective and cost-effective prescribing in line with NICE guidance, local formularies and practice policies.
• Contribute to repeat prescribing, medicines safety, clinical audits, quality improvement and prescribing reviews.
• Work collaboratively with community and hospital pharmacy teams and other healthcare professionals to ensure continuity of care.
• Maintain accurate clinical records and comply with professional, legal, confidentiality and information governance requirements.
• Participate in multidisciplinary meetings, clinical governance and service development.
• Maintain professional competence through continuing professional development and work within GPhC standards and scope of practice.

Job responsibilities

Experience
Essential

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Relevant experience in GP practice or a strong understanding of the clinical pharmacist role in general practice.
• Sound clinical knowledge of medicines optimisation, prescribing and management of long-term conditions.
• Excellent communication and interpersonal skills, with the ability to communicate effectively with patients, carers and healthcare professionals.
• Confident clinical decision-making skills and the ability to work autonomously within professional competence.
• Ability to identify and manage medication-related risks and promote patient safety.
• Good organisational skills and the ability to manage workload and competing priorities.
• Ability to work effectively as part of a multidisciplinary team.
• Commitment to patient-centred care, equality, confidentiality and safeguarding.
• Evidence of continuing professional development and commitment to maintaining clinical competence.
• Good IT skills and experience using electronic clinical/prescribing systems.

Desirable

• ndependent Prescriber qualification.
• Experience of conducting structured medication reviews and supporting patients with long-term conditions.
• Experience in medicines reconciliation, clinical audits and quality improvement.
• Experience working collaboratively with community pharmacy and secondary care.

Qualifications
Essential

• Essential
• Master of Pharmacy (MPharm) degree or equivalent pharmacy qualification.
• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Evidence of ongoing Continuing Professional Development (CPD).

Desirable

• Postgraduate qualification in Clinical Pharmacy, Primary Care Pharmacy or a related clinical discipline.
• Independent Prescriber qualification
• Relevant postgraduate training or accreditation in primary care/clinical pharmacy.
• Additional training in areas such as medicines optimisation, prescribing, long-term condition management or clinical assessment.
