We are seeking an experienced and enthusiastic Practice Nurse, 20 hours per week, to join our multidisciplinary clinical team. This is an exciting opportunity for a motivated nurse with expertise in long-term condition management, particularly diabetes and asthma, and a passion for delivering proactive, preventative healthcare within primary care. Experience in spirometry and respiratory disease management would be highly desirable.

The successful candidate will play a key role in delivering evidence-based nursing services, supporting patients to manage their health effectively and contributing to the continued development of high-quality clinical services. Working closely with GPs, Clinical Pharmacists, Healthcare Assistants and the wider multidisciplinary team, you will provide holistic care that improves patient outcomes and promotes wellbeing.

Main duties of the job
The post holder will work as an autonomous Practice Nurse, providing high-quality, evidence-based nursing care to the practice population and supporting the delivery of safe, effective and patient-centred services.

Clinical Responsibilities
Undertake chronic disease management clinics, including:

• Diabetes
• Asthma
• Hypertension
• Coronary Heart Disease
• Cardiovascular Disease primary prevention

Carry out annual reviews, monitoring and personalised care planning for patients with long-term conditions.

Provide advice, education and support to patients to encourage self-management and healthier lifestyles

Administer and promote national and local immunisation programmes, including childhood, influenza, travel and routine vaccinations.

Deliver women's health services including cervical cytology and contraception advice within competence.

Undertake wound care, dressings, treatment of minor injuries, and removal of sutures and staples.

Ensure accurate, contemporaneous and legally compliant clinical record keeping.

Work within professional scope of practice and recognise when referral to a GP or other clinician is required.

Strategic management & planning:

• Keep abreast of current affairs and identify potential threats and opportunities
• Contribute to Practice strategy; formulate objectives and research and develop ideas for future Practice development
• Work with Partners and Practice Manager in planning and setting strategic targets
• To undertake required tasks to meet the agreed aims and be wholly responsible for them
• Develop and maintain effective communication both within the Practice and with relevant outside agencies

Primary Responsibilities:

• Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
• Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
• Provide vaccination and travel medicine services, including childhood, adult and travel vaccinations
• Provide wound care - ulcer Doppler etc. to patients, including dressings
• Request pathology services as necessary

Process pathology results as required

Provide chronic disease clinics and Maintain skills and an interest in Diabetes, Asthma, COPD, Spirometry, Coronary Heart Disease, Hypertension, Stroke, TIA and Contraception.

• Delivering patient care as necessary, referring patients to secondary / specialist care as required
• Maintain accurate clinical records in conjunction with extant legislation
• Ensure read codes are used effectively
• Maintain chronic disease registers
• Implement and promote health during NHS Health checks
• Implement vaccination programmes for adults and children and travel
• Prioritise health issues and intervene appropriately
• Recognise, assess, and refer patients presenting with mental health needs to the appropriate services
• Support patients in the use of their prescribed medicines or over the counter medicines within own scope of practice
• Support the clinical team with all safeguarding matters, in accordance with local and national policies
• Deliver opportunistic health promotion where appropriate
• Maintain baby immunisation results records
• Maintain smears results logs
• Maintain emergency drug kit and log sheets
• Manage daily fridge temperatures and log sheets, and be aware of the cold chain procedures

Secondary Responsibilities:

• Develop, implement, and embed health promotion and well-being programmes
• Implement and evaluate individual treatment plans for chronic disease patients
• Assist GPs with minor surgery when required
• Chaperone patients where necessary
• Support the team in dealing with clinical emergencies
• Liaise with external services including specialist nursing teams and agencies to ensure the patient is supported appropriately vulnerable patients
• Delegate clinical responsibilities appropriately ensuring safe practice and the task is within the scope of practice of the individual
• Understand practice and local policies
• Act as the audit lead, effectively utilising the audit cycle
• Monitor and ensure the safe storage, rotation, and disposal of medicines
• Support junior members of the nursing team, providing guidance when necessary
• Participate in local initiatives to enhance service delivery and patient care
• Support and participate in shared learning within the practice
• Continually review clinical practices, responding to national policies and initiatives where appropriate
• Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis

Other tasks will include:

• Maintaining and cleaning equipment used by the nurses and GPs
• Maintaining Nurses rooms, stocking and rotating items as required
• Maintain general tidiness and cleanliness of nurses and treatment rooms
• Assisting with the ordering of vaccinations, dressings and equipment to maintain stock levels
• A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
• Attend and participate in any Practice meetings when required.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice team.

Attend and contribute to relevant internal and external meetings as required including practice clinical meetings, nurse team meeting and care planning meetings

Maintain a personal progress and training plan to keep up to date with current thinking and developments, as well as identifying own training requirements, attending appropriate courses

Promote evidence-based practice through use of latest research based guidelines

Monitor the effectiveness of their own clinical practice through quality assurance strategies such as the use of peer audit and review

Maintain professional registration, including revalidating as required

Work within the latest NMC Code of Professional Conduct

Record accurate consultation date in patients records in accordance with NMC guidance and other standards

Administering of injections as per local PGDs

Reviewing and signing PGS and keep up to date with revisions

Keep up to date with pertinent health policy and work with the practice team to consider the impact and strategies for implementation

Qualifications
Essential

• Registered Nurse with current NMC registration
• Nursing degree or equivalent experience
• Evidence of immunisation training and assessment of competence within the last two years
• Evidence of continuing professional development

• Minimum six months experience working within a primary care environment within the last two years
• Experience of chronic disease management
• Experience managing Diabetes, Asthma, Hypertension, Coronary Heart Disease and Cardiovascular Disease
