Description

Job summary
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, immunisations, cervical screening, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

The Practice nurse will also be responsible for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

Main duties of the job
The main duties of the role are:
  • Health promotion and wellbeing
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health
  • Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF
  • Provide wound care (ulcer/Doppler etc.) to patients
  • Provide immunisations (childhood, seasonal etc) to patients
  • Provide travel medicine services
  • Provide Cervical Cytology screening
  • Request and process pathology results as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read codes are used effectively
  • Chaperone patients where necessary
  • Assist GPs with minor surgery when required
  • Support the team in dealing with clinical emergencies and safeguarding matters.
  • Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual

About us
We are a premises-owning, five Partner, training practice with a growing list size, currently around 14,200. We have two Salaried GPs, a Clinical Pharmacist, an Advanced Care Practitioner, two Nurse Practitioners, three Practice Nurses, one Nursing Associate and one Healthcare Assistant, and a strong close-knit administrative team providing clinical support.

As well as the Practice staff, patients can also access additional roles including Social Prescribers, Adult Mental Health Workers and First Contact Practitioners through our BYTES Primary Care Network.

We have a strong reputation as a teaching and training Practice. We share our passion for Primary Care through providing placements for sixth form students, as well as teaching medical students, allied health professionals and GP Registrars. Each week we provide "clinical huddle" as well as "lunch and learn" for all staff, where we have covered topic areas such as menopause, dementia awareness and self-defence.

We are strongly embedded in our local community. Over the last few years, we have participated in Yarm Fun Run, organised and held community public health awareness days (for cancer awareness and CPR training), and held Open Days for school students interested in medical careers.

Person Specification
Experience
Essential
  • Experience of working as a Practice Nurse within a nursing team, ideally in a primary care environment.
  • Experience of infection prevention and control.
  • Experience of understanding the importance of evidence based practice.

Desirable
  • Experience of working in General Practice.
  • Experience of chronic disease management, wound care, cervical cytology, immunisation programmes, ECGS, phlebotomy, etc.

Qualifications
Essential
  • Registered Nurse

Desirable
  • Nurse Prescriber
