Job summary
We have an exciting opportunity for a Clinical Pharmacist to join the Ripon & Masham Primary Care Network (PCN). Whether you are newly qualified and looking to start your career in primary care, or an experienced Clinical Pharmacist seeking a new challenge, we would love to hear from you. At Ripon & Masham PCN, we are proud of our friendly, supportive and highly collaborative pharmacy team. Our team consists of experienced Clinical Pharmacists and Pharmacy Technicians who work closely together, sharing knowledge, supporting one another and building strong relationships with our GP practices. We provide regular peer support, clinical discussion and mentoring, creating an environment where pharmacists at every stage of their career can develop confidence, broaden their clinical skills and thrive.

Main duties of the job
  • Work collaboratively within the PCN pharmacy team and alongside GPs, nurses, and other healthcare professionals to deliver safe, effective, and evidence-based patient care.
  • Undertake Structured Medication Reviews (SMRs) for patients with complex medicines needs, including those with polypharmacy, frailty, multiple long-term conditions, and residents of care homes.
  • Optimise medicines to improve clinical outcomes, reduce medicine-related harm and support patients to get the best from their treatment.
  • Deliver medicine-related consultations, primarily by telephone, with face-to-face appointments where clinically appropriate.
  • Provide expert medicines advice to clinicians and practice teams, supporting safe and cost-effective prescribing.
  • Identify opportunities for deprescribing and medicines optimisation in line with national guidance and local prescribing policies.
  • Contribute to prescribing safety initiatives, clinical audit, and quality improvement projects across the PCN.

About us
Ripon and Masham Primary Care Network (PCN) consist of four GP practices serving the small city of Ripon, the market town of Masham and the surrounding rural communities. We share a commitment to joint working, whilst maintaining the continuity and identity that our communities value. This full-time role is primarily based across two practices, with the opportunity to provide cover at the remaining practices when required. The role is mainly based on site, with some flexibility for remote working where appropriate. Yorkshire Health Network (YHN) is the local federation for sixteen GP practices across the Harrogate and Rural areas, containing four PCNs in total. YHN and the PCNs work collaboratively to deliver a supportive structure which is focused on achieving the best from a joint management and clinical delivery setup for the benefits of patients across the region.

Job responsibilities
Clinical Practice
  • Undertake autonomous patient consultations.
  • Independently assess, diagnose, and manage patients within your competence.
  • Prescribe safely and effectively as an Independent Prescriber.
  • Undertake Structured Medication Reviews.
  • Optimise medicines for patients with complex needs.
  • Manage long-term conditions within your scope.
  • Support frailty, care homes, and end-of-life care.
  • Improve medicines safety.

Medicines Optimisation
  • Deliver evidence-based prescribing.
  • Implement NICE guidance.
  • Reduce inappropriate prescribing.
  • Lead deprescribing initiatives.
  • Support antimicrobial stewardship.
  • Optimise high-risk medicines monitoring.

Quality Improvement
  • Lead prescribing audits.
  • Analyse prescribing data.
  • Support QOF and Enhanced Services.
  • Deliver quality improvement projects.
  • Evaluate clinical outcomes.

Person Specification
Skills
Essential
  • Excellent consultation skills.
  • Clinical decision making.

Desirable
  • Presenting and teaching.

Experience
Essential
  • Long-term condition management or an interest in developing this.
  • Medication Review.

Qualifications
Essential
  • MPharm or equivalent recognised pharmacy qualification.
  • Current GPhC registration.

Desirable
  • Clinical diploma or equivalent postgraduate qualification.
  • Independent Prescriber qualification or working towards completion.
  • CPPE Primary Care Pathway or equivalent.
  • Membership of the Royal Pharmaceutical Society.
