Main duties of the job- Work collaboratively within the PCN pharmacy team and alongside GPs, nurses, and other healthcare professionals to deliver safe, effective, and evidence-based patient care.
- Undertake Structured Medication Reviews (SMRs) for patients with complex medicines needs, including those with polypharmacy, frailty, multiple long-term conditions, and residents of care homes.
- Optimise medicines to improve clinical outcomes, reduce medicine-related harm and support patients to get the best from their treatment.
- Deliver medicine-related consultations, primarily by telephone, with face-to-face appointments where clinically appropriate.
- Provide expert medicines advice to clinicians and practice teams, supporting safe and cost-effective prescribing.
- Identify opportunities for deprescribing and medicines optimisation in line with national guidance and local prescribing policies.
- Contribute to prescribing safety initiatives, clinical audit, and quality improvement projects across the PCN.
About usRipon and Masham Primary Care Network (PCN) consist of four GP practices serving the small city of Ripon, the market town of Masham and the surrounding rural communities. We share a commitment to joint working, whilst maintaining the continuity and identity that our communities value. This full-time role is primarily based across two practices, with the opportunity to provide cover at the remaining practices when required. The role is mainly based on site, with some flexibility for remote working where appropriate. Yorkshire Health Network (YHN) is the local federation for sixteen GP practices across the Harrogate and Rural areas, containing four PCNs in total. YHN and the PCNs work collaboratively to deliver a supportive structure which is focused on achieving the best from a joint management and clinical delivery setup for the benefits of patients across the region.
