Job summary
We are looking for an enthusiastic and motivated Associate Practitioner to join our Physical Health and Wellbeing Team, supporting the delivery of physical health and wellbeing clinics and clozapine services across Bradford District Care NHS Foundation Trust's community mental health services.

This is an exciting opportunity to play a key role in improving the physical health outcomes of people living with serious mental illness. You will work alongside nurses, clinicians and the wider multidisciplinary team to provide physical health monitoring, health promotion, wellbeing interventions and lifestyle support for service users across community mental health services.

The successful candidate will be passionate about reducing health inequalities, promoting healthy lifestyles and supporting people to achieve better physical and mental wellbeing. You will have the opportunity to develop specialist skills in physical health assessment, health coaching, venepuncture, ECG recording and long-term condition monitoring within a supportive team environment.

Main duties of the job

• To provide physical health monitoring and wellbeing interventions to people requiring physical health checks or when initiating new medication, within Community Mental Health and Clozapine services.
• To deliver quality care and support which enables clients to be supported throughout their care pathway whilst promoting health and wellbeing.
• To work alongside both registered and unregistered staff running clinics, supporting colleagues and facilitating groups.
• To communicate effectively at all times within the team, with other health care professionals and with the client and their family.

Qualifications
Essential

• Educated to NVQ 3/4-Health and Social Care
• Care certificate

Desirable

• Grade C or above in GCSE Maths and English (or equivalent).
• Foundation degree in Health and Social Care or related subject (or equivalent experience).

Experience
Essential

• Significant experience of working in a relevant health or social care setting
• Experience of personal care and related procedures
• Experience of dealing with members of the public

Knowledge
Essential

• Understand the workings of the NHS and how this interfaces with community working.
• Understanding of the needs of people who experience serious mental illness.

Desirable

• Knowledge of nursing procedures and practices

Skills

• IT skills

• Experience of using electronic patient record systems in a healthcare setting
