Job summaryThe Quality and Patient Safety is responsible for ensuring that systems and processes are maintained in support of a systematic approach to reporting, assessment, investigation, analysis, evaluation and management of quality and patient safety throughout the Division.
This includes support to clinical management teams (Speciality Triumvirates) in the management of patient safety incidents / formal and informal complaints when they occur and identification and promotion of good practice in risk management and risk reduction to ensure shared learning. A key role is the provision of highly competent professional advice regarding the protection of patients, staff, and visitors where appropriate. Patients and their safety are at the centre of all that we do.
Main duties of the jobThe Quality and Patient Safety is required to have a minimum of 2 years' experience working within a patient safety/ governance role, ideally within a clinical environment. The post holder will work closely with members of a specific Speciality Governance team/s within the Division, promoting the adverse incident policy and complaints procedure: monitoring compliance, and supporting the specialty governance teams in their day-to-day recording, management, investigation, follow up and learning from adverse incidents/ formal and informal complaints as well as risk management. The post holder will work with other team members in delivering appropriate training programmes for all groups of staff.
The post holder will be expected to demonstrate an enthusiastic approach and proactively contribute to service improvements. They will support the lead investigator of enhanced learning under the Patient Safety Incident Response Framework (PSIRF) and complaint responses. The post holder will produce a monthly quality and safety newsletter to feedback learning from incidents, best practice and highlight alerts or essential information which needs to be shared with clinical staff.

Job responsibilities- To support the Divisional Quality and Patient Safety Manager (Team Manager) in the design of systems and processes to ensure compliance with Care Quality Commission standards and collation of evidence as required, and the monitoring and delivery of Divisional action plans (e.g. setting up Board to Floor visits).
- Supported by the Team Manager assume responsibility for the systems of governance relating to complaints and patient experience, including the production of trend analysis and outcome reports for Divisional and Trust forums, for speciality groups. To play a key role in report writing to include providing qualitative and quantitative data on incidents, risks and complaints to the Speciality groups/ Division, liaising with the Team Manager.

Training will also include delivery of Enhanced learning i.e. After Actions reviews (AARs), Multiprofessional Safety Reviews (MPSR) etc
- To actively promote the Trusts Incident Management Policy and procedures throughout the Division, as the basis for reporting and mitigating patient safety incidents.
- Provide expert advice/ support in relation to the management and investigation of complaints related to that area.
- To be a Datix key administrator and have systems in place to ensure tracking of clinical incidents.
- To be able to run ad hoc queries from Datix and deliver these in an appropriate format for users.
- To maintain Datix Dashboard for the Speciality groups/ division.
- Monitor incident and risk trends and report these to Divisional lead for patient safety and service leads.
- To support the Team manager to monitor and oversee the Datix risk entries and work to manage the Divisional and Departmental risk registers, including liaison with risk owners to ensure consistency of scoring and timely completion of reviews and actions on the risk register.
- Support the Team manger to prepare for any external accreditation with regard to Risk Management and improving Patient Safety.
Person SpecificationKnowledge and ExperienceEssential- Minimum 2 years' experience in clinical practice or relevant equivalent experience
