Job summary
An exciting opportunity has arisen in the Patient Safety Team for a Patient Safety Learning Response Lead to support how the Trust identifies, analyses and shares learning from patient safety events. The postholder will work collaboratively with clinical and non-clinical teams, using subject matter expertise to interpret findings and contribute to recommendations that strengthen organisational learning and patient safety.

The postholder will also support the development of a positive, restorative and learning-focused culture, producing learning bulletins, safety alerts and contributing to patient safety policy and guidance, including PSIRF-related documents.

Main duties of the job

• Contribute to provision of advice, support and guidance when serious patient safety events occur
• Produce learning bulletins, safety alerts and notices and ensure they are communicated across the Trust
• Collate, analyse and interpret and report on patient safety related qualitative and quantitative data and information, identifying and analysing trends or themes
• Work collaboratively with the Network Quality teams, legal team and complaints team to ensure triangulation of all incidents within the Trust and analyse findings to highlight quality and safety priorities ensuring that quality, risk and safety related data contributes to the whole system view of care
• The post holder will be communicating with staff and patients regarding areas of patient safety learning and presenting detail from analysis of safety data for high level forums and groups
• Communicate complex and sensitive information to staff, patients, senior leaders and external agencies, and present learning at high-level forums and groups
• Develop programme plans to progress learning and improvement work, ensuring delivery within agreed timeframes.

Job responsibilities
Key responsibilities include:

• Supporting the organisation's response to serious patient safety incidents and learning reviews.
• Promoting a restorative, just and learning culture in line with PSIRF principles.
• Producing and coordinating patient safety learning bulletins, alerts and communications.
• Contributing to the development and implementation of patient safety policies, procedures and guidance.
• Analysing safety information from incidents, complaints, legal claims and other intelligence sources to identify themes and improvement opportunities.
• Presenting learning and patient safety insights to senior groups and forums across the Trust.
• Leading and supporting improvement programmes that strengthen patient safety and quality of care.

Person Specification
Education / Qualifications
Essential

• To be educated to degree level, or have equivalent level qualification plus short courses and/or experience to post-grad diploma level
• Evidence of continuing education and professional development specific to patient safety, clinical risk management, quality improvement and quality assurance
• Professional and specialist knowledge in patient safety, safety culture and PSIRF

Desirable

• Use of Ulysses Risk/ Incident Management Systems
• Experience within a governance related area (e.g. clinical risk management, incidents, complaints, clinical effectiveness, quality and safety)

Experience
Essential

• Working with sensitive/confidential information involving patient safety
• Experience with data analysis and presentation of data analysis
• Experience of managing multiple projects, deadlines and fluctuating workloads
• Use of governance management systems / databases

Knowledge
Essential

• Professional and specialist knowledge in Patient Safety, Safety culture and PSIRF
• Patient Safety theory and methodology. Learning response and learning framework methodologies.
• Quality/ continuous improvement methodology

Desirable

• A working knowledge of the Duty of Candour.

Skills and Abilities

• Excellent report writing skills. Demonstrable negotiation skills to ensure challenging groups work together.
• Track record of successful policy, procedures and guideline development. Able to use Microsoft Office software.
• Ability to interpret highly complex and sensitive data and create further sustainable improvement processes from the analysis
• Excellent verbal, written and interpersonal communication skills

Work Related Circumstances

• Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
