Role OverviewAs a Clinical Case Manager, you will be responsible for coordinating rehabilitation programmes for clients, liaising with medical professionals, and ensuring the highest standards of care.
Key Responsibilities- Manage a caseload of clients with complex needs
- Develop and implement personalised rehabilitation plans
- Coordinate with multidisciplinary teams
- Monitor progress and adjust plans as necessary
- Maintain accurate and up-to-date records
Requirements- Registered healthcare professional (e.g., Nurse, Physiotherapist, OT)
- Experience in case management or rehabilitation
- Excellent communication and organisational skills
- Ability to work independently and as part of a team
