Job Summary
CLCH Merton Adult Community Neuro Rehabilitation Team - Lead Occupational Therapist. We are seeking an experienced Specialist Occupational Therapist with expertise in stroke and neurological rehabilitation to join our Integrated Community Neuro Rehabilitation Team in Merton. Our service follows the national model for community neuro rehabilitation and provides person-centred care across Early Supported Discharge (ESD), Community Stroke Rehabilitation and Community Neuro Rehabilitation pathways. Working within a multidisciplinary team, you will support people recovering from stroke and those living with a range of neurological conditions, including Motor Neurone Disease (MND) and Parkinson's Disease (PD). We are looking for an adaptable and motivated Occupational Therapist with excellent communication and interpersonal skills, and experience of stroke and neurological rehabilitation. You will contribute to service development, support the ongoing enhancement of rehabilitation pathways, and help shape high-quality, evidence-based care while further developing your clinical leadership skills.

Main Duties of the Job
As a member of the CLCH Merton Community Neuro Rehabilitation multidisciplinary team, you will provide a comprehensive occupational therapy service to residents within Merton, supporting patients across the Early Supported Discharge (ESD) pathway and wider community stroke services. Key responsibilities include:

• Assessing, managing and treating a caseload of patients within the stroke pathway.
• Delivering evidence-based occupational therapy interventions in line with professional standards, clinical guidelines and best practice.
• Working collaboratively within the multidisciplinary team to support individuals in achieving their rehabilitation goals and maximising participation in daily activities.
• Building effective relationships with referring services to support timely and appropriate referrals.
• Leading occupational therapy assessments and interventions, prioritising workload to meet patient needs.
• Providing supervision and support to junior staff, Rehabilitation Assistants and Occupational Therapy students.
• Contributing to service development, quality improvement, audit, evaluation and Continuing Professional Development (CPD) activities.
• Supporting the ongoing development of stroke and neurorehabilitation pathways and promoting best practice across the service.

Person Specification
Education/Qualification
Essential

• Diploma/degree in Occupational Therapy
• Current HCPC registration
• Evidence of up to date CPD portfolio
• Attendance at specialist advanced courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology and stroke

Desirable

• Educated to Masters Level in Neuro Occupational Therapy or equivalent combination of post graduate diplomas

Experience
Essential

• Experience post registration as a Band 6 Occupational Therapy in neurology/stroke rehabilitation
• Post-registration experience of working in a community rehabilitation setting at a senior level.
• Experience working within a multi-disciplinary team
• Experience working at a junior level in a wide range of specialised clinical areas including, stroke and neurology, Cognitive impairments outpatients
• Experience in supervising and appraising junior therapists and support workers.
• Experience in supervising Occupational Therapy students

Desirable

• Experience working with voluntary organisations

Skills & Knowledge
Essential

• Skills in the assessment and treatment of adults with complex neurological conditions and multi-pathologies
• Knowledge and understanding of complex medical conditions including, neurology, multi-pathologies
• Knowledge of the relevant clinical guidelines
• Advanced communication and report writing skills, in both verbal and written format
• Basic IT skills - Microsoft Word, Power point, Excel
• Advanced knowledge and understanding of the neuro rehabilitation process

Desirable

• Knowledge and experience in research and audit

• Must have a car and valid driver's license in the UK
