Occupational Therapy and /or Physiotherapy provides assessment and treatment for patients who have or are likely to fall in our Community.
The team provide assessments, a programme of care for vulnerable adults living in the community.
The Band 6 OT or PT contributes to service development and improvement. The Band 6 would provide advice and guidance, junior staff. The post holder will work within the Integrated Falls Service - mainly in Broad Green Centre. There are also Falls prevention Groups held in Croydon Hospital and in some areas around the Borough.
Interview date TBCMain duties of the job- The postholder will be responsible for assessing the functional needs of adults in their homes and in a community setting, in line with all Croydon Health Services (CHS) Trust's policies and procedures.
- To manage a caseload of clients with complex needs, using evidence based/client centred principles to assess, plan, implement and evaluate interventions.
- To provide leadership for junior staff through supervision, competencies and appraisal.
- To participate in the planning, development and evaluation of Therapy services within a designated area/team, holding responsibility for defined projects.
- To contribute to the maintenance and development of the trust Therapy service.
- To regularly supervise OT or Physio students on practice placements.
- To work with assigned by your manager and with the Team

Person SpecificationEssential- HCPC Registered as an Occupational Therapist
- Good understanding of NHS and adult social care
- Experience supervising and mentoring others
- Significant social services and community Occupational Therapy experience
- Experience of service improvements
- Significant post qualifying professional development
- Knowledge and experience of care pathways in this specialist field
- Evidence of good organisation skills
