Job summaryWe have a fantastic opportunity for Senior Community Nurses to join our nursing teams within both Greater and North PCN in Derby City Community Services. We are in a position to offer a variation of hours ranging from 22.5-30hrs This is an ideal opportunity for a registered nurse with varied community experience who wants to develop their leadership and caseload management skills in a supportive environment working alongside a District nurse.
Main duties of the jobAs a successful applicant you will require community nursing experience, knowledge of caseload management, with a high standard of community nursing knowledge and clinical skills. You will also require exceptional people skills and communication skills, with an ability to work as part of a multi - disciplinary team
You will be provided with support tailored to your needs to drive forward high quality care and team work; including a thorough induction programme, regular one to ones and peer supervision.
You will provide effective evidenced based care to a defined population/Primary Care Network. This is achieved through effective delegation, case load management, monitoring of care standards, leadership and the performance/ line management of team members.
You will assess, plan, implement and evaluate individualised patient care, utilising your professional judgement at all times.
You will provide clinical leadership and line management to less experienced Nurses, and other members of the multidisciplinary team in the delivery of high quality care.
You will contribute to the continuous improvement of community nursing within their Place, linked to the 10 year NHS plan.
