Job summary
An exciting opportunity has arisen for you to join our Tissue Viability Team.

The service needs an exceptional, dynamic and self- motivated clinician with wound care expertise with a Registered Nurse qualification to provide Tissue Viability Specialist support and advice for patients in both Derbyshire and Derby city.

You will be committed to quality improvement of the Tissue Viability Service across the Trust by contributing towards the development and delivery of education and training that will motivate staff to improve practice and to deliver safe and effective care.

You will be a visible and accessible role model, have excellent communication and leadership skills and be committed to improving outcomes for our patients.

Main duties of the job
  • To promote the Tissue Viability Vision and Strategy, providing support to clinicians and health care workers to prevent and manage wounds by adhering to Tissue Viability Policies and Procedures.
  • Act as a role model for excellence in TV practice and the 'DCHS Way'.
  • To actively engage in delivering and identifying areas for service improvement.
  • To provide Tissue Viability input in the prevention, monitoring and management of wounds, including pressure ulcers, to patients and health care workers in DCHS services, and other settings as required both face to face and by telephone.
  • Liaise with multi-disciplinary agencies to support the ongoing management of patients.
  • To provide support and advice to staff, patients and members of the general public in any matters relating to Tissue Viability.
  • To work closely with the wound clinics to offer additional Tissue Viability support directly to the clinics.

Person Specification
Education, Qualifications and training
  • NMC Registered RGN
  • Evidence of education or training in Tissue Viability or Wound Care
  • Significant experience at a band5/6

Desirable
  • Recognised teaching qualification

Experience and Knowledge
  • Experience at band 5/6
  • Knowledge on National/Local Tissue Viability initiatives, guidance and policies

Desirable
  • Experience of community working
  • Case load management

Skills and Attributes
  • Leadership and communication skills
  • Ability to prioritise work loads
  • Basic information technology skills (Power point, Word and Outlook)

Aptitude and Personal Qualities
