Job summary
We currently have an opportunity available for a Trainee Enhanced Clinical Practitioner (tECP) or Enhanced Clinical Practitioner (ECP) within our Erewash TeamUp Frailty Service. You will be an enthusiastic, committed, dynamic and forward-thinking individual who is passionate about providing an outstanding and progressive health care service to the Frail patients. These patients will be predominately within a care home setting, but may also be within individual homes.

Main duties of the job
This is a great opportunity to join a collaborative and supportive multidisciplinary team. Progression from tECP to ECP (Band 7) would be supported following successful completion of the ECP modules and your ePortfolio.

Initially, the post holder would be based at Ilkeston Community Hospital, working alongside our existing team of Community GPs, ACPs and ECPs. There will also be an requirement to rotate to our Long Eaton team to support both your development and service needs. The successful candidate would be employed by Derbyshire Community Health Services (DCHS); however, the service is delivered in partnership with Erewash Health Partnership.

Due to the nature of the role, applicants must be able to travel independently across the locality and have access to a suitable vehicle for business purposes.

Qualifications
Essential

• O levels in Maths and English or equivalent
• Professional qualification such as NMC or HCPC
• Non Medical Prescribing module L6/7
• CDM/APACS or equivalent

Experience

• System One experience
• Experience in working with frailty team

• Community experience
