Job summaryWe are looking for an enthusiastic Band 5 Physiotherapist to join our team. To provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy and dignity. To undertake assessment of patients / service users care needs and develop programmes of care/Rehabilitation as part of the wider multi-disciplinary care team. To provide advice to patients/ service users and carers regarding their care. To plan, implement and review health improvement programmes in a range of settings. Provide supervision, mentorship, and clinical support to students and junior staff in the team. This is a full time post 37.5 hours per week. The post holder may be required to work flexibly to meet the needs of the service.
Main duties of the jobTo provide leadership and work within the Integrated Community Rehabilitation Team, specifically, with adults who have been referred to the team, through provision of a high standard of physiotherapy specialist assessment, and core assessment details of other multidisciplinary skills. Develop care and/or rehabilitation programme, to promote and sustain independence/well-being, for the individual or carer in the relevant setting, which may be a community, residential or health care environment. To assess, plan, implement and evaluate treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team. Plan, implement and review health improvement programmes in a range of settings. Recognise, assess and manage risk across the immediate and wider working environment ensuring statutory requirements are met. To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.

Person SpecificationKNOWLEDGE, SKILLS AND TRAININGEssential- Diploma /BSc/BA Physiotherapy
- Membership of the relevant Professional Body
Desirable- Membership of Special Interest Group
JOB SPECIFIC EXPERIENCEEssential- Demonstrable knowledge of assessment and therapeutic interventions in area of practice
- Understanding of professional ethics and their application in practice
- Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
- Evidence of recent professional development in an up to date portfolio
- Understanding of how audit, evidence-based practice, and research can improve service development
Desirable- Recent previous experience within a comparable role
- Documented evidence of continuing professional development
- Knowledge of partnership working
Additional requirementsEssential- Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively
Information TechnologyEssential- Able to evaluate data and produce reports using Microsoft Excel and Word
- Experience of using electronic patient / service user record systems
BUSINESS TRAVELEssential- Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business
- Holds current Full UK driving License
