We are expanding our Rehabilitation Community Therapy service and are looking for a dedicated, compassionate and professional Band 5 Physiotherapist who wants to make a genuine difference in the lives of people.
The service has recently undergone an exciting restructure to help support and empower our patients even further with specialities for service development being fundamental in our approach.
What We're Looking For- Qualification in physiotherapy

Main duties of the job- Provide person-centred rehabilitation therapy in clients' homes and community settings
- Support clients to develop daily living skills, mobility, confidence, and independence
- Deliver structured intervention plans and monitor progress
- Work collaboratively with physiotherapists, OTs, nurses and wider MDTs
- Facilitate group-based rehabilitation, wellbeing sessions, or community outreach and in-reach
- Promote safety, dignity, and empowerment in every aspect of care

Job responsibilitiesWork within and adhere to the Chartered Society of Physiotherapists Code of Professional Values and Behaviour (2011), The Health Care Professionals Council (HCPC) standards of practice, The Trust relevant policies and guidelines.
The post holder is an autonomous practitioner, responsible for provision of assessment, treatment, intervention and discharge planning of patients referred to the service.
Responsible for managing a caseload of patients with a variety of conditions including chronic, disabling and life limiting illnesses.
Carry out assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention. Specifically assessing issues related to the barriers which prevent the individual carrying out activities of daily living e.g. mobility, moving and handling, functional restrictions, activity tolerance, hand eye coordination, positioning, cognitive impairment, social and psychological issues, environmental restrictions.
Assist senior therapists in planning, co-ordinating, delivering and evaluating the therapy service.
Educate other health care professionals, multi-agency professionals, therapy students and apprentices within the specialty.
Work in the community as a lone practitioner in line with the Trust Lone Worker Policy.
Person SpecificationEducation and QualificationEssential- Degree or Diploma in Physiotherapy
- HCPC Registration
Desirable- Accredited fieldwork educator
Experience and KnowledgeEssential- CPD diary including evidence of reflective practice and undergraduate experiences
- Theoretical and clinical knowledge underpinning physiotherapy practice.
- Broad range acquired at undergraduate/newly qualified level.
- Experience of working within the NHS at undergraduate/newly qualified level.
Skills and AbilityEssential- Good communication, planning, organisational and time management skills
- Basic IT skills
- Ability to carry out moderate physical effort.
- To work in an environment with competing demands.
- Motivation, drive and enthusiasm for delivering a quality service as part of the Community Therapy team
- Legally entitled to work in UK
- Ability to pass Occupational Health requirements
- Current full drivers licence valid for UK use
- Access to a car for work purposes
