Job summaryWe are seeking an enthusiastic and experienced Band 6 Nurse to join our multidisciplinary team as a Frailty Practitioner. This is an exciting opportunity to work within the Weymouth and Portland Integrated Community Service to support Frailty Hospital At Home and the Complex Frailty MDT function.
The post holder will play a key role in delivering high-quality, person-centred care for patients living with frailty, recognising and managing complex/deteriorating patients and to prevent inappropriate admission to acute trusts.
This will involve working across community settings, providing specialist clinical input to optimise independence, reduce hospital admissions, and improve patient outcomes. You will act as an autonomous practitioner while working collaboratively within the Integrated Neighboured Team and wider services to deliver coordinated care.
You will contribute to the development of frailty pathways and support service improvement initiatives, ensuring care is evidence-based, safe, and responsive to the needs of the local population.

Person SpecificationKnowledge skills and trainingEssential- Registered Practitioner
- Evidence of recent professional development
- Experience of working in community
Desirable- Physical examination qualification
- Experience at band 6 level, or above
- Able to use own initiative and appropriate risk taking
- Experience and knowledge of working with frail patients
Job specific experienceEssential- Experience of managing change
- Demonstrable knowledge of assessment and therapeutic interventions in area of specialism

- Recent previous experience within a comparable community role
