Job summary
Downs Way Medical Practice is looking for an enthusiastic and experienced paramedic to join our multidisciplinary primary care team. This is an Additional Roles Reimbursement Scheme (ARRS) role, supported through Garden City Primary Care Network, and based across all three Downs Way Medical Practice sites: Istead Rise, Shorne and Summerhouse.

The role combines same-day clinical work with proactive support for frail, housebound and complex patients. You will work with appropriate clinical autonomy, supported by GP's, the practice team and wider PCN colleagues. The post would suit a paramedic who enjoys continuity of care, collaborative working and helping patients remain safely at home whenever clinically appropriate.

Main duties of the job
  • Undertake planned and urgent home visits for housebound, frail and acutely unwell patients.
  • Assess and manage patients presenting with minor illness within your competence and agreed scope of practice.
  • Provide timely assessment of falls, deterioration, confusion and reduced mobility, using structured observations and clinical judgement.
  • Support safe admission avoidance through assessment, treatment, safety-netting, follow-up and coordination with community services.
  • Contribute to Comprehensive Geriatric Assessments and proactive care planning for people living with frailty or complex needs.
  • Support care homes and respond appropriately to urgent clinical requests.
  • Participate in neighbourhood multidisciplinary team working with community, social-care and voluntary-sector colleagues.
  • Contribute to advance-care planning and ReSPECT discussions within your competence and local governance arrangements.
  • Recognise safeguarding concerns, including neglect, self-neglect and unsafe home environments, and act in line with local procedures.
  • Maintain clear, contemporaneous clinical records and escalate promptly when a patients needs fall outside your competence.

Downs Way Medical Practice delivers care across Istead Rise, Shorne and Summerhouse and works closely with Garden City Primary Care Network and neighbourhood partners. Job responsibilities
Clinical care
  • Assess undifferentiated patients through focused history, examination and observations; formulate and document a safe management plan.
  • Recognise deterioration and red flags, using tools such as NEWS2 where appropriate, and escalate promptly to the most suitable service.
  • Provide evidence-based treatment, advice, follow-up and safety-netting within agreed competence and prescribing authority.
  • Verify expected deaths where trained, competent and authorised under local policy.

Frailty and neighbourhood care
  • Identify frail or complex patients who may benefit from proactive support and contribute to holistic assessment and anticipatory care planning.
  • Coordinate person-centred care across the three Downs Way Medical Practice sites and with PCN, community, care-home, social-care and voluntary-sector colleagues to support independence and avoid unnecessary admission.

Professional practice
  • Work within HCPC standards, Downs Way Medical Practice and PCN policies and agreed competencies, recognising limits and seeking support appropriately.
  • Prioritise a varied workload safely and maintain clear, contemporaneous records, confidentiality and information governance.
  • Maintain training and CPD and contribute to safeguarding, audit, quality improvement and team learning.

Person Specification
Qualifications
Essential
  • HCPC Registration
  • Evidence of Continuous Professional Development

Experience
Essential
  • Experience of dealing with vulnerable and elderly/frail patients.
  • Experience of working in a community care setting

Desirable
  • Experience in a Primary Care and Community settings
