Job summary
The Integrated Community Nursing team is looking for an enthusiastic Registered Nursing Associate to join the team. Applicants should have previous experience at Band 4 within a hospital or community setting, and demonstrate transferable clinical skills as detailed in the job description and personal specification; however training would be available where required.

Due to the nature of the role it is essential that applicants are able to travel effectively and independently on a daily basis to and from the base, clients' homes and other venues around the geographical area whilst transporting equipment. Main duties of the job
The nursing associate has a breadth of knowledge across the lifespan of patients and across the fields of nursing, providing holistic and person-centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses who work within the sphere of nursing and care.

Nursing associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.

Job responsibilities
Main Duties and Responsibilities
  • Provide and monitor care in the community setting: Provide compassionate, safe and effective care and support to people in a range of care settings.
  • Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.
  • Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment.
  • Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
  • Recognise and report any situations, behaviours or errors that could result in poor care outcomes.
  • Planning: Working alongside senior health professionals in planning the workload and completing tasks on Sys1/Malinko
  • Participating in co coordinating patients and workload

  • Put people first and provide nursing care that is person centred, safe and compassionate.

Person Specification
Experience
Essential
  • Experience of working in teams
  • Experience of patient-centred care
  • Experience of health coaching and promoting independent living
  • Competent with Sys1 and competencies relevant to community district nursing.

Desirable
  • Previous experience of working within the community setting/District Nursing service
  • Evidence of involvement in support / development of less experienced staff

Skills & Knowledge
Essential
  • Technical/Work-based Skills
  • Ability to communicate with members of the public and health and social care providers.
  • Ability to deal with a non-routine and unpredictable workload and individual patient contact.
  • Ability to manage own workload.
  • Knowledge of when to seek advice and indirectly expressed feelings.
  • Has a range of mechanisms for dealing with stress, can recognise when to use them and does so?
  • Listens to personal comments without becoming defensive.

Desirable
  • Experience with Malinko/Civica or willingness to train.

Qualifications & Training
Essential
  • Foundation Level Degree: Nursing Associate Qualification
  • NMC Registered Nursing Associate
  • Completion of End Point Assessment for applicants who have undertaken an apprenticeship route.
  • GCSE Grade A-C English and Maths or Level 2 in Maths and English
  • Educated to a minimum level 5 Foundation degree level specific to nursing associate practice.

Desirable
  • Care Certificate or willingness to train.
  • Related health and social care qualification (Level 3)
