Main duties of the jobThe post holder will be responsible for triaging all referrals coming into the West Essex CCC service across Unplanned care, Integrated Community Care Team (ICCT), and Transfer of Care Hub (TOCH), with the primary aim of strengthening existing triage team, improving patient flow, and supporting neighbourhood working. The role will support the current band 7 triage coordinator.
The post holder will be responsible for communicating with various acute hospitals, community teams, neighbourhood teams including primary care and external parties.
Provide therapy-led clinical decision-making to avoid unnecessary admissions, facilitate early discharge, support same-day responses and select appropriate pathways for community based referrals for the integrated neighbourhood teams.

Job responsibilitiesClinical/Professional Duties- 1. To triage incoming referrals for all therapy services across west essex and the inpatient beds across West, accepting or declining them based on inclusion/exclusion criteria and service specifications.
- 2. To develop links, and complete in-reach, with staff in the acute hospitals to inform potential referrers of the service, support identification of suitable clients and liaise with staff regarding outcomes of triaging and/or assessments.
- 3. To communicate with the bed managers regarding outcome of inpatient referrals
- 4. To monitor the pathway of current patients ensuring they are on track for their planned discharge date and participate in discussion with Clinical and Service Leads if patient is requiring an extension.
- 5. To monitor and supervise Band 4 Associate Practitioner in their role
- 6. To maintain clinical records as per Trust and professional guidelines and to write timely correspondence to colleagues, health, social services and other colleagues, reflecting specialist knowledge.
- 7. To continue to work clinically within professional standards and clinical guidelines, promote best practice, through active participation in continuing professional development (CPD) and to undertake all aspects of clinical duties as an autonomous practitioner
To support service development and by participating in any audit or research being undertaken by the service relating to this job role eg referrals, patient pathway.
Person SpecificationQualificationsEssential- Registered healthcare professional (e.g. RN, OT, Physiotherapist, Paramedic or equivalent) with current professional registration
Experience and SkillsEssential- Strong skills in clinical assessment, risk management, and decision-making
- Proven clinical experience in community, urgent care, or triage settings
- Knowledge of planned and unplanned care pathways
- Excellent communication skills, with the ability to engage effectively with patients, carers, and professionals
OtherEssential- Uk driving licence
