Job summaryThe post holder will provide advanced and expert level of practice (including non-medical prescribing) within the adult Community Mental Health Team leading to improved patient outcomes.
Main duties of the jobThe post holder will support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit. They will be an integral part of the MDT supporting clinical decision making and supporting the service pathways, developing nurse led clinics and providing brief interventions. The post holder will also support with providing a seamless pathway into & out of the community mental health team (CMHT) once interventions / goals have been achieved, to promote recovery and self-management, as clinically appropriate. The ACP will hold a case load and will lead assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures. The post holder may additionally lead on practice and service developments. The post holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice. To support improving workforce knowledge and practice through the contribution to a CMHT education and training programme across the community teams within Bolton. To participate in the Bronze on site / Bronze on Call rota. The core hours are Monday to Friday 9-5, although some flexibility, including weekend / BH working may be required.
