Job Summary
An opportunity has arisen for an Advanced Clinical Practitioner (Frailty/OPMH) to join our dedicated teams based on Herm & St Brelades Wards, Alderney Hospital.

The inpatients services are for Older Persons with a organic or functional mental health diagnosis.

We are looking for a desire to engage, motivate and innovate within this specialised and autonomous role. You will have advanced skills and be able to assess, diagnose and formulate a clinical management plan for the patients with complex, chronic illnesses or requiring end of life care.

Working together with Primary Care, Community Services, Social Services and the Voluntary Sector to provide enhanced and pro-active care.

Applicants should have a broad experience working within community or urgent care settings at a senior level and knowledge of working within the field of older peoples care. You must have proven experience of working across the pillars of Advanced Practice and are expected to have successfully completed the MSc Advanced Clinical Practitioner programme and be a non-medical prescriber.

Main duties of the job
To provide high quality person centred care which always considers peoples' safety, privacy and dignity using advanced clinical knowledge that is evidence based.

As part of a multi-professional team, the post holder will practice autonomously, whilst being accountable and self directed in line with the relevant code of professional conduct. Plan and manage complete episodes of care; undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations; making complex clinical decisions regarding service usermanagement and clinical outcomes. To provide expert clinical advice and leadership ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision making skills.

To lead as a driver for change, to monitor and improve standards through supervision, evidence based practice, clinical audit, research and education. To work strategically, contributing to the planning and development of integrated services

We actively support learners, including those undertaking the ACP programme, there will be an expectation that the post holder will provide clinical supervision and support.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

You will be required to undertake a Prevention and Management of Violence and Aggression course, and it is a condition of your employment that you successfully complete all aspects of this course; if this is not achieved you will not be able to commence working in the post.

Person Specification
Knowledge, Skills & Training
Essential
  • Registered health practitioner
  • Relevant full Masters degree or demonstrable extensive equivalent experience in the relevant specialty
  • Evidence of recent evidence based professional development in an up to date portfolio demonstrating leadership, education and service development achievements including links to national work and standards
  • Registered non-medical prescriber (if professionally appropriate)
  • Clear and demonstrable evidence of advanced, autonomous practice in the management of patients / service users
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training
  • Knowledge and understanding of the Trust's strategies

  • Able to undertake clinical supervision and systematic peer caseload review of colleagues on an individual or group basis.

  • Confident in the use of computer systems, spread sheets, databases, data collection and coordination, word processing and report writing.

Personal Qualities/Attributes
Essential
  • Evidence of demonstrating the Trust's values and behaviours
  • Evidence of skills in diplomacy, negotiation and influencing
  • Effective communicator able to communicate highly complex and sensitive information where there may be barriers to understanding

Business travel

  • Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
