Job summary
The post holder will support the delivery of GMMH's Shared Care Protocols for psychotropic medicines.

As part of the shared care MDT, the post-holder will undertake prescribing activity until stability (from 3 months to 12 months), specialist reviews and follow-up consultations.

The postholder will be an integral member of the MDT, providing high quality pharmaceutical advice, both internally and externally, and to patients and carers.

Main duties of the job
  • To prioritise patients with complex medication needs for medication histories, medication reviews and clinical input.
  • To provide pharmaceutical care for patients providing a clinical pharmacy and prescription monitoring service ensuring safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients.
  • To act as an expert source of pharmaceutical advice for health and social care staff, patients and carers.
  • To provide a clinical pharmaceutical opinion when medical information is conflicting.
  • To recommend, substantiate and communicate pharmacological treatment options to consultant medical staff, GPs and junior medical staff.
  • To recommend individualised, complex care-plans encompassing all relevant mental, physical and social care needs.
  • To act as a pro-active and reactive source of medicines information for members of the MDT.
  • To act as a non-medical prescriber in line with service need and within the scope of the trust NMP policy.
  • To receive, interpret and act on highly complex clinical information from a wide variety of conflicting sources.
  • To counsel patients and carers regarding choice of medication, likely benefits and side effects on an individual basis.
  • To practice in accordance with Code of Ethics of General Pharmaceutical Council.
  • To provide a face-to-face/phone prescribing clinic

Person Specification
Education & Qualifications
Essential
  • Pharmacist registered with the General Pharmaceutical Council.
  • Non- Medical Prescribing Qualification
  • Post graduate diploma in clinical / psychiatric pharmacy (or equivalent experience)
  • 4 Year master's degree with 1 year competency-based training with final exams

Desirable
  • Credentialed membership of the College of Mental Health Pharmacy

Experience
Essential
  • Mental Health experience
  • Supervision of staff
  • Delivery of training at Post Graduate Level
  • Experience of multidisciplinary working
  • Experience in educating patients and carers

Desirable
  • Primary Care experience

Knowledge & Skills
Essential
  • Have a knowledge of clinical governance, clinical audit and risk management
  • Have up to date knowledge of mental health therapeutics and services
  • Have the ability to interpret and communicate highly complex data
  • Have the ability to manage change
  • Have high level numeracy and computer literacy skills
