Main duties of the jobAs a GP Practice Nurse, you will provide high-quality nursing care to patients across a range of clinics and services. Your duties will include administering immunisations and vaccinations, cervical screening, wound care, chronic disease management, health promotion, and supporting patients with long-term conditions. You will carry out clinical assessments, maintain accurate patient records, work collaboratively with the wider multidisciplinary team, and contribute to delivering safe, effective, and patient-centred care within the practice

Job responsibilitiesThe Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to patients within the GP practice. The post holder will work as part of the multidisciplinary team to provide safe, effective, and compassionate care, with a primary focus on wound care and dressings, cervical screening, chronic disease management, and immunisations.
The Practice Nurse will promote health and wellbeing, support the prevention and management of long-term conditions, and contribute to the efficient delivery of primary care services while maintaining the highest standards of clinical practice.
Key Clinical Responsibilities- Provide wound assessment, treatment and dressings using evidence-based practice.
- Perform cervical screening in accordance with the NHS Cervical Screening Programme guidelines.
- Administer routine, seasonal and travel immunisations and vaccinations in line with national and local guidance.
- Deliver chronic disease management clinics, including monitoring and supporting patients with long-term conditions such as diabetes, asthma, COPD and hypertension (according to competence and training).
- Undertake health promotion and provide lifestyle advice to patients, supporting disease prevention and self-management.
- Carry out health assessments and appropriately escalate concerns to a GP or other healthcare professional.
- Recognise and respond appropriately to medical emergencies within the practice.
- Provide advice, support and education to patients and their carers.
Professional Responsibilities- Maintain accurate, contemporaneous and confidential clinical records using the practice's clinical system.
- Adhere to the NMC Code and maintain professional registration and revalidation requirements.
- Work within the scope of professional competence and seek support when required.
- Participate in clinical audit, quality improvement initiatives and service development.
- Maintain mandatory training and continue professional development.
Infection Prevention and Control- Maintain high standards of infection prevention and control at all times.
- Ensure the safe management, storage and disposal of clinical waste and sharps.
- Follow practice policies relating to cleaning, decontamination and safe working practices.
Communication and Teamworking- Work collaboratively with the GP Partners, Paramedic, reception team and medical secretaries to deliver coordinated patient care.
- Communicate effectively with patients, relatives and colleagues.
- Participate in practice meetings and contribute to the ongoing development of services.
General Responsibilities- Comply with all practice policies, procedures and NHS guidance.
- Maintain patient confidentiality and comply with GDPR and information governance requirements.

Person SpecificationEssential CriteriaQualifications- Registered Nurse (Adult) with current NMC registration.
- Evidence of continuing professional development.
- Cervical Screening Sample Taker qualification or willingness to obtain.
- Immunisation and Vaccination training.
Experience- Experience working as a Practice Nurse or in a primary care setting.

- Experience of managing patients with long-term conditions, including chronic disease reviews.
- Experience of delivering health promotion and providing lifestyle advice to patients.
- Experience of maintaining accurate and contemporaneous electronic clinical records.
- Experience of working effectively as part of a multidisciplinary team while also managing a clinical workload independently.
- Experience of maintaining high standards of infection prevention and control in a clinical environment.
