Job summaryWe are seeking a part time Practice Nurse to join our experienced Nursing team here at Hawthorn Surgery. The role includes long-term condition management, cytology, vaccinations, health screening, health promotion. Experience of working in General Practice is essential.
Main duties of the jobThe successful applicant must be a Registered Nurse and have current NMC registration. The main duties are to provide general nursing services and support to the GPs, Nurses and patients at Hawthorn Surgery.
About usHawthorn Surgery has a list size of 6900 patients and is a GP Training Practice. We work from Wortley Beck Health Centre, located in Leeds 12.We are a System One practice and our friendly team includes our Practice Manager, GP Partners, Salaried GPs, Nurses, Healthcare Assistant, administrative and reception staff. We are also supported by our PCN team which includes pharmacists, physiotherapists and Social Prescribers.
During the last patient survey, we were voted 7th best practice in Leeds. This is a key achievement of ours and one we hope to keep continuing in the future.
Job responsibilitiesWe are looking for a part time Practice Nurse. The successful candidate will hold current NMC registration. We are looking for a nurse who is enthusiastic, organised and confident to work unsupervised in a treatment room. An enhanced DBS check will be required for this post.
The post is not open to candidates from outside the UK as we are not able to offer visa sponsorship.
The successful applicant may at times be asked to cover for colleagues annual leave and other absences.
Our ideal candidate would be somebody who :
- Would maintain accurate and confidential patient records using SystemOne.
- Participate in Infection Prevention & Control procedures including monthly, quarterly and annual audits.
- Ensure clinical equipment is maintained and appropriately stocks
- Support quality improvement initiatives, clinical audits and service development.
- Participate in practice meetings, training and continued professional development (CDP)
Duties include:
- Assess, plan, implement and evaluate patient care
- Manage patients with Long-term conditions including : Diabetes, Asthma, COPD, Hypertension and Coronary Heart Disease
- Administer Vaccinations and Immunisations in accordance with National Guidelines.
- Administer Injections and medications under Patient Group Direction of prescribing qualifications where applicable.
- Perform Cervical Screening
- Undertake wound care, dressings, suture removal or clip removal.
- Perform to the highest standard of infection control.
- NHS Health Checks
- New Patient Health Checks
- Lifestyle advice and Health Promotion
- Venepuncture,ECG's, Blood Pressure Monitoring, Urinalysis and other clinical procedures.
- Adhoc Duties as needed - Home Visits
Person Specification- Experience
- Experience of working in General Practice
- Excellent communication skills and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of Safeguarding, Infection Control and Clinical Governance
- Qualifications
- Qualified Nurse with current NMC registration
