Job SummaryWe are looking to find a perfect fit for our current team - a full or part time, experienced General Practice Nurse, who wants to work in a high achieving Practice. We expect the candidate to be able to demonstrate a high standard of clinical excellence with experience and qualifications in long term condition management.
The ideal candidate will also be forward thinking, a great team player, have excellent communication skills, and the ability to work collaboratively with the nursing and HCA team at the practice, led by the Nurse Manager.
Main duties of the jobBoth full-time and part-time hours will be considered. Our full-time staff work 36 hours per week over 4 days, Monday to Friday, with a day off during the week. About usWe are a friendly, supportive team situated in the beautiful Ribble Valley, with a main site in Whalley and a branch site in Sabden.
5 Partners, 6 Salaried GPs, ANP, 3 Nurses, NA, 2 HCAs, and a large reception and admin team, we are also a long-standing training practice for GP training. We also have Housebound Nursing Team of nursing professionals who work with our care home and housebound patients.
We have a PCN team that work with our patients, including a Pharmacist, Pharmacy Techs, GP Assistant, Care Co-ordinator and a physio. We also have an onsite dispensary at both sites - to give all of our patients a good quality experience at the surgery.

Job responsibilitiesJob Summary - please see attached for the full Job DescriptionTo be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
Person SpecificationExperienceEssential- Experience of working in a primary care environment
- Experience of working autonomously
- Experience of working as a practice nurse or community nurse
- Experience of infection prevention and control measures
- Experience in leading long-term conditions management and quality improvement within a PCN

- Wound care/removal of sutures and staples
