Main Duties of the JobThe successful candidate will lead and deliver high-quality nursing care within a busy GP practice environment, with a particular focus on long-term condition management including diabetes and respiratory disease. The role will also involve childhood immunisations, cervical screening, health promotion and supporting the wider multidisciplinary team to provide safe, effective and patient-centred care. Job ResponsibilitiesMain Duties of the Job
The successful candidate will be responsible for delivering chronic disease management clinics, particularly in diabetes, asthma, COPD, hypertension and cardiovascular disease. They will undertake annual reviews and support patients in the management of their long-term conditions and self-care.
The role will include administering childhood and adult immunisations in accordance with national guidance and local protocols, alongside undertaking cervical cytology screening in line with the NHS Cervical Screening Programme.
The post holder will also carry out health promotion, screening and NHS Health Checks, provide wound care, injections and general practice nursing duties, and maintain accurate and contemporaneous clinical records using SystmOne.
The successful candidate will be expected to recognise, assess and appropriately escalate safeguarding concerns or deteriorating patients, while supporting the practice in achieving QoF and other contractual quality frameworks.
Person SpecificationExperienceEssential- Previous experience working within General Practice.
- Substantial experience managing long-term conditions, particularly diabetes and respiratory disease.
- Experience undertaking cervical screening and childhood and adult immunisations.
- Experience working independently within a multidisciplinary clinical team.
- Experience delivering patient-centred care within a busy clinical environment.
- Experience maintaining accurate clinical records and using clinical systems effectively.
Desirable- Desirable criteria include experience using SystmOne, supporting QoF and enhanced services, undertaking NHS Health Checks and health promotion activities, and mentoring or supporting staff or students.
QualificationsEssential- Registered Nurse with current NMC registration.
- Evidence of continuing professional development.
- Qualified and competent in cervical cytology screening.
- Qualified and competent in childhood and adult immunisations/vaccinations.
- Good understanding of infection prevention and control standards.
Desirable- Desirable criteria include experience using SystmOne, spirometry experience and previous involvement with QoF or enhanced services.
- Experience supporting students, mentoring colleagues or participating in service development and quality improvement initiatives would also be advantageous.
