Job summaryAn exciting opportunity has arisen for a Practice Nurse to join our very stable and long standing nursing team. The post holder will work as an autonomous practitioner within the multidisciplinary team, delivering high-quality, evidence-based nursing care to the practice population. The post holder will manage acute and long-term conditions, promote health and wellbeing, and support the delivery of safe, effective and patient-centred services. Howden Medical Centre is located in the lovely market town of Howden, with a very friendly patient population. We are very near to Goole, with easy access from Hull and the villages in between. We would be looking to recruit for 30 hours per week. We are a growing practice. With a current list size of 8300, which is expected to almost double in the next ten years, and a new practice building being planned, this could bring opportunities for the right person. Main duties of the job- Undertake comprehensive patient assessments and formulate appropriate treatment plans
- Manage and monitor patients with long-term conditions
- Provide treatment room services including wound care, venepuncture, ECGs, cervical cytology, injections and immunisations
- Deliver NHS Health Checks, health promotion and preventative care initiatives
- Request, interpret and act upon investigations and blood test results within scope of practice
- Provide health education and support patients to self-manage their conditions
- Identify and manage patients at risk of developing long-term conditions
- Provide clinical advice and support to the multidisciplinary team
- Deputise for the Lead Nurse/Nurse Manager when required
- Participate in clinical meetings, audits and quality improvement initiatives
- Assist in the development and review of clinical policies, protocols and procedures
- Ensure compliance with CQC standards, infection prevention and control policies and safeguarding procedures
- Maintain accurate patient records and uphold confidentiality and information governance requirements
Professional Responsibilities- Maintain current NMC registration and practice in accordance with the NMC Code
- Participate in appraisal, mandatory training and continuing professional development
- Work in accordance with Practice policies and Health and Safety legislation
About usHowden Medical Centre is a Systmone Practice with Medical Students, Registrars, we have trained HCAs and supported individuals through Nurse Training. We are involved in Research. We have very strong and long standing GP, Nursing, Reception and Admin Teams as well as the PCN staff who work within the practice. We strive to maintain an open and inclusive culture where everyone's ideas are shared, respected and cultivated. Although we have different professional teams, we work as one cohesive team where every team member is equally respected. We are a very family orientated practice and strive to maintain wherever possible the balance between the pressures or work and home life.
