Main duties of the jobYou will develop excellent working relationships with nursing and clinical teams across the Trust to ensure the effective utilisation of hospital beds. Provide a visible, clinical leadership presence on behalf of the Site Management team - energising, inspiring and empowering nursing teams and clinical staff in pursuit of achievement of effective patient flow and discharge, minimising patient delays and driving a positive patient experience and outcome where possible. Promote an ethos of joint working, collaboration, a 'can do' problem solving approach to aid patient flow. Job responsibilitiesYou will work with Ward Managers, Matrons, other clinical staff, Clinical Directorate Managers and other Trust colleagues to ensure beds are managed effectively to meet patient needs. To give clinical advice and support to staff, as required, in the admission and discharge of elective and emergency admissions and in the transfer of patients within the hospital. To have the responsibility for giving advice or signposting to the most appropriate information source, information and clinical guidance/assistance on nursing issues out of hours. Demonstrate professional behaviours at all times. Communicate with patients, relatives and staff demonstrating a professional courteous manner. Accountable and responsible for raising concerns when care is compromised. Negotiation with trust colleagues, senior clinicians and Local Authority colleagues, using tact and diplomacy to agree actions for discharge planning. Liaison with external organisations e.g., NHS Trusts to negotiate safe and timely transfers/repatriations of patients to the most appropriate speciality or locality organisation/facility for their needs. Maintain patient comfort, dignity and privacy at all times. Liaise with patients, relatives, multi-disciplinary team and other healthcare . Person SpecificationExperienceEssential- Registered Nurse/AHP Evidence of first level or equivalent experience
- Evidence of Continuous professional development Recognised teaching/ mentorship course
- Management qualification or relevant experience
- Highly specialist knowledge of NHS operational processes, targets and standards
- Detailed knowledge of NICE Guidelines related to discharge process.
Desirable- Recognised management qualification to postgraduate level
- Knowledge and understanding of a range of issues across the NHS and NHS patient agenda
- Detailed knowledge of the Department of Health discharge process and proven track record of implementation
