Job summary
The Burnley Community Mental Health Team are a friendly, dedicated, supportive and flexible team, who strive to provide the best care possible with the resources we have in our area.

We are seeking to appoint a Band 6 Community Mental Health Practitioner with a passion for community mental health, who will help us deliver this to our Service Users.

We are looking for a registered mental health practitioner to be part of our team. You need to be able to work as part of a multidisciplinary team and have experience in assessing, planning, implementing and reviewing care needs.

You will also need to have experience of risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.

You must have significant post qualification experience in clinical work with service users experiencing a range of mental health problems.

Main duties of the job
  • To undertake mental health assessments including risk assessments within a multi-disciplinary assessment team. Working closely with the Consultant Psychiatrist, thus ensuring that there is a multidisciplinary team approach
  • To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.
  • To provide a fast, responsive service to assess individuals needs for access into mental health services
  • Providing a communication structure between GPs , IAPT and secondary MH services in LCFT, non-statutory service to ensure the highest possible standards of patient care
  • To provide mentoring and support for others

Qualifications
Essential
  • Recognised Core Mental Health Professional Qualification (e.g. RMN, SW, OT)

Knowledge
Essential
  • In-depth knowledge of mental health problems In-depth knowledge of issues that impact upon people with mental health problems
  • Detailed knowledge of the impact of risk and protective factors that affect people with mental health problems
  • Knowledge of medication prescribed for management of mental health issues

Experience
Essential
  • Post qualification experience of clinical work with service users experiencing a range of mental health problems
  • Experience of delivering evidence based interventions in a time-limited framework
  • Evidence of assessment across a broad range of mental health problems
