Job summaryMendip Vale Medical Group (MVMG) are looking for further Advanced Practitioner to join our existing team in our Sea Mills Surgery, as we expand in our friendly and innovative practices serving over 95,000 patients within our 11 functional patient sites in North Somerset, Bristol, South Gloucester and Bath, Swindon & WIltshire.
As an Advanced Practitioner you will exercise advanced clinical expertise, acting within your professional boundaries, providing advanced clinical skills and advice.
If you are passionate about improving patients health outcomes through the demonstration of the sound understanding of the issues related to the identification, assessment, diagnosis, treatment, and management of patients within your care; as well as working collaboratively with the multidisciplinary surgery team this maybe the job for you.
Main duties of the jobTo deliver a high standard of patient care as an Advanced Practitioner in general practice, using specialist knowledge, and a broad and in-depth theoretical knowledge base utilising autonomous clinical skills.
Assess, diagnose, plan and implement care for patients presenting with both acute and chronic conditions, within the Surgery and the patients own homes, integrating both drug and non-drug based treatment methods into a management plan.
To implement and operate according to the four pillars of advance practice and in line with the requirements of the NMC/HCPC code of conduct.

Job descriptionJob responsibilitiesTo provide highly skilled Advanced Clinical assessment, diagnosis, advisory, triaging and onward referral for patients presenting with acute and chronic conditions, ensuring the services are delivered in line with current trends in health care delivery, following current evidence based practice and skills in keeping with the Advanced Practice Frameworks.
Taking professional responsibility to flexibly, independently and safely manage your allocated Primary Care caseload supported by a GP / Lead AP mentor.
To maintain a high level of expertise within this specialty, ensuring implementation of evidence-based practice and auditable standards accountable for decisions and actions through NMC/HCPC registration.
To ensure timely and effective communication within the multi-disciplinary team and Practice Management team as relevant.
Person SpecificationExperienceEssential- Experience in management long term conditions e.g. asthma, COPD, diabetes, CHD.
- Evidence of working autonomously and as part of a team.
- Experience of use of a medical software package.
- Understanding of evidence based practice.
- Proven ability to evaluate the safety and effectiveness of own clinical practice.
QualificationsEssential- Registered General Nurse/Paramedic (Currently registered with the NMC or HCPC).
- MSc in Advanced Practice (or training towards)
Desirable- Non-Medical Prescribing
- Current IRMER training and competency in requesting appropriate radiological investigations
