Job summaryWe are looking for a compassionate, motivated and enthusiastic Community Healthcare Support Worker to join our Children and Young People (CYP) Autism Service.
This is a new, exciting role and opportunity to work as part of a multidisciplinary team supporting autistic children and young people and their families across South Staffordshire. You will be working predominantly alongside our nurses but also with our Occupational Therapists, Psychologists, Speech and Language Therapists and other professionals to deliver person-centred, evidence-based care that promotes independence, wellbeing and positive outcomes for our CYP and their families.
The successful candidate will have excellent communication skills, a caring and flexible approach and a passion for improving the lives of children and young people with autism. Previous experience working with children and young people or individuals with neurodevelopmental conditions is desirable.
Main duties of the jobTo work as part of the CYP Autism Team for the South Staffordshire area. The post holder will work in a person-centred way providing support to our nurses working with CYP with Autism. This may involve carrying out specific health focussed activities, observing behaviour or gaining information from staff or families. Information gained will play an important part in the formulation of the difficulties experienced by the CYP and their families. The post requires travel across the area covered by the community team.

All clinical work will be agreed with, planned (based on a clinical formulation) and executed under the supervision of the appropriate health care professional. All clinical activities to be documented clearly in the care plan with supervision provided by the health care professional responsible for the care plan.
- To work with CYP who have very complex needs which may include moderate- severe learning disabilities, autism or additional mental illness.
- To contribute to the overall assessment, planning, implementation and review of individual CYP and to lead specific assessments under the supervision of qualified staff.
- To be part of a person-centred service where the needs of the individual will determine the type of intervention. The post holder will work across a variety of settings which are relevant to the service user.
- To carry out work in the community sometimes in conjunction with other members of the multi-disciplinary team.
- To complete physical health investigation tasks such as pulse, blood pressure monitoring, temperature, height and weight.
- To work as part of a team and in conjunction with other professionals, statutory agencies and any associated service providers for the service user. This may include the Intensive Support Team.
- To develop a professional relationship conducive to the client reaching their full potential, within stated goals.
- To be involved in relevant meetings/reviews related to the service users treatment/service.
- To receive clinical and managerial supervision to maintain best practice. This will include discussion of caseload, reflective practice and the ability to receive and act upon constructive feedback. Clinical supervision should be received from relevant professionals in relation to caseload.
- To take part in training and development as agreed at appraisal. To undertake own continuing professional development by demonstrating an ongoing reflective learning style in the workplace. This should be clearly and regularly documented in a personal portfolio.
- To participate in all mandatory and statutory training deemed necessary for this role in line with the Trusts training matrix.
- To use an appropriate range of teaching media to assist with clinical interventions, teaching other staff, carers and service users, students and other disciplines where appropriate.
- To ensure that all Trust policies and procedures are implemented and adhered to within the area of practice.
- To undertake other associated duties linked to the job and grade, to reflect the evolving nature of the service.
- To adhere to Trust and professional guidelines on confidentiality and to communicate any issues to the team leader.
- To be responsible for own and others health and safety, reporting all issues to the line manager and working within the Trusts health and safety policies and the Health and Safety at Work Act.
- To participate in own appraisal by completing preparation documentation and actively partaking in the appraisal process.
- To respect the individuality, values, culture, religion and diversity of clients contributing to a service to meet their needs.
- Participate in local, national and organisational clinical governance activities as required including audits, surveys, evidence-based practice and research projects.
