The closing date is 24 July 2026Humber and North Yorkshire Integrated Care Board (857)Job summaryThe Children and Young Peoples Continuing Care (CYPCC) Team are looking for experienced registered nurses (RN Child, RNLD/RNMH, RN with PHN qualification) who are passionate about improving care for children and young people with complex needs.
You will need to be able to work independently and take a lead role in providing needs led and child centred assessment, development & clinical oversight of care packages for children and young people with complex needs referred for and receiving Children and Young People's Continuing Care. Experience of working in the field of childrens complex health/palliative care/learning disability/mental health is essential. We are particularly keen to attract nurses with experience of working with children/young people/adults with autism and/or intellectual disability and behaviours that challenge in order to develop our team skill mix due to an increase in referrals in this area.
We have various hours available; our CYPCC Nurses are predominantly remotely based and work across the Humber and North Yorkshire region. Where possible CYPCC Nurses will be allocated cases within the closest distance to their home, however willingness to travel across the 6 places (North Yorkshire, York, Hull, East riding, North and North East Lincolnshire) is a requirement. This will be discussed with the manager at interview stage.
Main duties of the jobOur CYPCC Nurses undertake evidence based, holistic needs assessments following the National Framework for Children and Young People's Continuing Care. The assessment process is a collaborative approach with the child and family at the centre, and extensive input from multi-agency professionals.
Utilising specialist nursing knowledge you will signpost to commissioned NHS services, identify gaps in health care provision, develop and commission comprehensive needs led packages of care in partnership with the child/young person, their family, and professionals across health, social care and education.
The post holders will provide specialist health advice and contribute to complex case management alongside colleagues in health, social care, and education. You will take an active role in the Education, Health and Care planning (EHCP) process; Care, Education and Treatments Reviews (CETRs) and multi-agency meetings with local authority Children with Disabilities Services. You will work under the guidance of our clinical Team Leaders and CYPCC Strategic, Operational, and Quality Lead, with access to regular clinical and safeguarding supervision, and opportunities for training and development.

Job responsibilities- To undertake comprehensive assessments of need for children with complex health needs, utilising specialist health knowledge and experience.
- Always maintain a person centred and multi-agency approach to needs assessment and care package development.
- To work in partnership with all relevant stakeholders in the provision of continuing health care packages to enable the child or young person to attain their optimum life experience and outcomes.
- To facilitate difficult and challenging conversations and communicate highly sensitive and complex information using high level communication skills to a range of audiences including medical/non-medical professionals, parents/carers, children/young people.
- To use advanced analytical skills in continuing health assessments for children in agreeing levels of care, funding and developing bespoke packages of care within agreed eligibility and criteria.
- To support parents/carers and young people to hold a personal health budget where eligible and desired.
- To review at least annually packages of care, outcomes, and personal health budgets, ensuring they continue to work effectively for the child and their family.
- To meet statutory duties under the SEND Code of Practice, engaging in the Education, Health, and Care planning process to ensure alignment of EHCP and Continuing Care.
- To attend SEND decision makings panels as a health representative or to present continuing health care assessments for peer review.
- To support professionals across education, health, and care services by offering timely, effective specialist health support and information.
- Ensure children and young people are safe and remain safe.
Person SpecificationQualificationsEssential- Registered Nurse currently registered with the NMC.
- Degree or Diploma level qualifications or equivalent experience
- Evidence of continued professional development.
- Knowledge of governmental policy in relation to the wider health care system
- Ability to update knowledge of various acts and statutory guidance such as the Mental Health Act, Mental Capacity Act, Valuing People, Safeguarding etc.
- Knowledge of risk management and clinical governance.
Desirable- Childrens Community nursing degree/willingness to undertake course or relevant equivalent course.
- Working knowledge of the current National Framework for Continuing Healthcare and Continuing Care
- Working knowledge of personal health budgets/direct payments
- Knowledge of responsibilities of the Local Authority in the context of NHS CHC/CC
ExperienceEssential- In order to undertake the duties of this post it is expected the post holder will have demonstrable broad post-registration experience in a relevant field of nursing e.g. complex health/palliative care/CAMHS/Learning Disability.
- Demonstrable post registration experience at Band 5 or above
- Experience of assessing, planning, implementing, and evaluating care.
- Experience of working collaboratively across organisational boundaries.
- Experience of managing own caseload and working autonomously
- Experience of communicating sensitive, complex and potentially distressing information appropriately to different audiences.
- Experience of complex case management within a multi-agency setting
Desirable- Experience of the National Framework for NHS CHC/CC
- Experience of the National Framework for NHS PHB
Skills and AttributesEssential- Ability to engage and work effectively with relevant agencies and multidisciplinary professionals at all levels.
