Job summary

We are looking for Band 5 community nurse who will be responsible for delivering a high standard of nursing care to patients in a community setting.

This post is a permanent position, working 19 hours per week.

The successful candidate will be able to work independently, assessing and providing care to individuals with various health needs. This will include those with chronic illnesses, post hospitalisation care, wound management, and palliative care (this list is not exhaustive).

The role involves collaborating with other healthcare professionals and the wider MDT, supporting patient recovery, and promoting self-care in negotiation with patients, their carers and families.

Main duties of the job
  • Assess, plan implement, and evaluate patient care in community settings.
  • Provide wound management including dressings, compression therapy, and other related interventions.
  • Administer medications, including injections.
  • Support patients with chronic disease management (e.g. diabetes, respiratory conditions).
  • Deliver End-of-Life care and support patients with palliative care needs.
  • Health education and self-care facilitation for patients to promote their independence.
  • Conduct initial assessments and regular reviews of patients health conditions.
  • Maintain accurate and up-to-date clinical records.
  • Ensure adherence to infection control and health and safety protocols.
  • Monitor patients for any changes in their condition, responding appropriately to emergencies and changes.
  • Refer patients to other healthcare professionals when necessary.
  • Work closely with GPs, physiotherapists, occupational therapists, social workers and other healthcare professionals to ensure holistic care.
  • Provide support and advice to patients families or carers, ensuring they are involved in the care process.
  • Attend Multidisciplinary team meetings and contribute to care planning.
  • Engage in continuous professional development (CPD) to keep update with the latest nursing practices and guidelines.
  • Supervise and mentor junior nurses or nursing students when required.
  • Participate in audits, quality improvement projects and research to improve community healthcare delivery.

Job responsibilities
Job Purpose
  • To provide nursing care to a variety of patients in their own home.
  • To work as part of the integrated health and social care teams in the community, providing holistic care to patients. This nursing care includes acute and chronic wound management, medication administration, catheter care, palliative care, long term conditions and self-care.
  • To provide holistic and individualised care to patients and carers as working autonomously and as part of a team.
  • To regularly take responsibility for care and workload management in the absence of the Team Leader or deputy.

Dimensions
  • To provide nursing care for house bound patients who are registered with a Nottingham City GP.
  • To participate actively in the services training strategy, including mandatory training and essential training and any other training if identified necessary for this role.
  • To participate in continuing professional development as relevant to maintain NMC registration requirements.
  • To work across the Nottingham City Community Nursing Teams as required.
  • To supervise and support student nurses on placements with the community nursing teams.
  • To supervise unregistered nurses who have been delegated patient care.

  • Promote and maintain record taking practice in line with the CityCare and NMC and professional standards.
  • Liaise with individuals and groups in a wide variety of settings and situations.

  • To be responsible for the development of own knowledge and clinical practice and recognises ones own limitations.
  • Act as a mentor to a variety of nursing students.
  • Participate in the development of other team members.
  • Participate in the supervision framework as a supervisee and supervisor.
  • Maintain a portfolio to demonstrate continuing professional development.
  • Contribute to the implementation of service developments.
  • Participate in the Personal Development process and take responsibility for reaching agreed development objectives.
  • Attend mandatory training.
  • Utilise enhanced knowledge and skills to promote continuous service improvements.
  • Promote and maintain best practice that ensures the health safety and security of self, patients and colleagues in accordance with the policies procedures and guidelines of NCCP.
  • Practice in accordance with Safeguarding legislation and guidelines.
  • In the absence of District Nurse Team Leader or deputy takes responsibility for the needs of patients and carers on the community nursing caseload and ensures that appropriate delegation and supervision of work is carried out in accordance to the skills and abilities of team.

Person Specification
Experience
Essential
  • Up to date clinical knowledge including wound care, assessment and management of continence, and palliative care.

Desirable
  • Postgraduate experience in a primary and / or a community care setting.
  • Either post registration experience or completion of a community management placement.
  • Evidence of working in a team.

Qualifications
Essential
  • RN qualification on part 1 or part 2 of the Nursing and Midwifery Council register.

  • Able to work weekends on a rota basis
  • Full driving licence, own transport and the ability to travel between locations.
