Practice Nurse with Extended Skills - DiabetesThe closing date is 30 August 2026Job summaryAn exciting replacement role has opened for a motivated, professional, and caring Practice Nurse with a Diabetes qualification to join our supportive multi-disciplinary team. Working across our two site practice, we provide high-quality care to a diverse population of 16,000 patients.
We are looking for a dedicated professional who thrives in a team environment and can demonstrate excellent, patient-centred care with compassion and professionalism. The successful candidate will work alongside the rest of the clinical team to manage chronic disease, while also supporting general practice nursing duties, using SystmOne for all clinical record-keeping.
Main duties of the job- Collaborative Diabetes Care: Work alongside our current Diabetes Nurse to deliver comprehensive assessment, treatment, and lifestyle advice for patients with diabetes.
- General Nursing Care: Deliver core treatment room services, including but not limited to; immunisations, cervical screening, and wound management.
- Clinical System Utilization: Maintain accurate, comprehensive, and contemporaneous patient records using SystmOne.
- Multidisciplinary Collaboration: Work collaboratively within our multi-disciplinary team (including GPs, ANPs, Practice Nurses, TNA, HCAs and AHPs) to support patient care pathways
- Quality Frameworks: Support the practice in achieving the Quality Outcomes Framework indicators and local quality initiatives related to diabetes.

Person SpecificationExperienceEssential- Proven experience delivering general practice nursing duties within a UK primary care setting.
- Experience in conducting structured diabetes reviews, lifestyle interventions, and managing stable diabetes regimes.
- Sound clinical knowledge of current diabetes guidelines (NICE) and Quality and Outcomes Framework (QOF) indicators.
- Competence in core treatment room duties (e.g., adult/child immunisations, wound care, venepuncture).
- Professional awareness of clinical governance, patient safety, and confidentiality protocols.
Desirable- Experience with complex diabetes management, such as GLP-1 therapy or assisting with insulin initiation.
QualificationsEssential- Registered General Nurse (RGN) with a valid, current NMC Registration (PIN).
- Accredited specialist qualification or certification in Diabetes Management.
- Evidence of continuous professional development (CPD) relevant to primary care nursing.
Desirable- Independent Nurse Prescribing qualification (V300).
- Accredited training in other long-term conditions (e.g., Asthma/COPD, Hypertension).
- Cervical Cytology (Smear) training and current sample taker code.
