Main duties of the job
The post holder will provide high-quality, evidence-based medical care to patients in a primary care setting. Duties include consulting with patients, diagnosing and managing a wide range of health conditions, prescribing medications, referring to specialist services where appropriate, and contributing to the ongoing management of chronic diseases. The role also involves supporting preventative health measures, maintaining accurate clinical records, and working as part of a multidisciplinary team to deliver safe and effective patient care.

Job responsibilities
The post holder will work as a Salaried GP within the practice, providing high-quality, patient-centred general medical services to our registered patient population. The successful candidate will work collaboratively with the practice team and wider multidisciplinary team to provide safe, effective and compassionate care.

The role will involve the assessment, diagnosis and management of a wide range of acute and chronic conditions, together with preventative healthcare, health promotion and appropriate referral to other services.

Key Responsibilities
The post holder will:

• Provide face-to-face and remote consultations as appropriate.
• Assess, diagnose and manage a wide range of clinical presentations.
• Provide appropriate treatment and management of acute and chronic conditions.
• Develop management plans in partnership with patients, taking account of their individual needs and preferences.
• Prescribe safely and appropriately in accordance with current clinical guidance, local policies and professional standards.
• Review and manage patients with long-term conditions and complex healthcare needs.
• Provide preventative healthcare, health promotion and lifestyle advice.
• Undertake appropriate clinical examinations and arrange relevant investigations.
• Review and act appropriately on laboratory results, imaging, correspondence and other clinical information.
• Provide appropriate follow-up and safety-netting advice to patients.
• Make appropriate referrals to secondary care, community services and other healthcare professionals.
• Liaise effectively with hospitals, community services and other external agencies involved in patient care.

Clinical Care
The post holder will provide comprehensive general medical care to patients of all ages within the scope of their competence.

This will include the management of undifferentiated and established illness, assessment of new symptoms, management of long-term conditions and appropriate preventative care.

The GP will use clinical judgement to determine appropriate investigation, treatment, monitoring, referral and follow-up. Patients should be involved in decisions about their care wherever appropriate, with clear communication regarding treatment options and safety-netting.

The post holder will recognise situations requiring urgent or emergency intervention and take appropriate action.

Multidisciplinary Working
The post holder will work closely with GPs, nurses, healthcare professionals, administrative staff and other members of the wider practice team.

The GP will contribute to effective communication and coordination of patient care and will work collaboratively with colleagues to ensure that patients receive appropriate and timely support.

The post holder will respect the skills and expertise of other members of the team and contribute to a positive, supportive and professional working environment.

Clinical Governance and Patient Safety
The post holder will contribute to maintaining high standards of clinical governance and patient safety within the practice.

The post holder will work collaboratively with colleagues and external agencies where necessary to ensure appropriate safeguarding arrangements are in place.

Communication
The post holder will communicate effectively and professionally with patients, relatives, carers, colleagues and external healthcare professionals.

The GP will provide information in a clear and appropriate manner and will demonstrate sensitivity and respect when discussing potentially difficult or sensitive issues.

The post holder will support shared decision-making and encourage patients to take an active role in managing their health.

Confidentiality and Information Governance
The post holder must maintain patient confidentiality at all times and comply with relevant legislation and practice policies relating to data protection and information governance.

All patient information must be handled securely and appropriately. Clinical records should be accurate, complete, contemporaneous and maintained in accordance with practice requirements.

Professional Development
The post holder will maintain their professional registration and undertake continuing professional development appropriate to their role.

The GP will keep their clinical knowledge and skills up to date and participate in appraisal and revalidation requirements as applicable.
