We are looking for 0.8 to 1 whole time equivalent- enthusiastic and highly motivated clinical pharmacists to join our team and work with our existing Senior Clinical Pharmacist, Clinical Pharmacists and Pharmacy Technicians. We aim to create a clinical pharmacist team to embed the role within our member practices. Background
Citrus Health Primary Care Network delivers healthcare services to a population of
45,228. This is a unique opportunity to help co-create the future of the pharmacy workforce within the Primary Care Network.
The successful candidate will have a higher degree in pharmacy and be a registered GPhC Independent Prescriber. They will also be expected to demonstrate completion of the national CPPE ARRS training pathway. The postholder will undertake face-to-face medication reviews for patients with polypharmacy, particularly older people, care home residents and those with multiple co-morbidities.
The role includes managing long-term conditions, working directly with patients to assess and treat conditions, and promoting self-care. The successful applicant will work as part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, medicines reconciliation following transfer of care, and systems to support safer prescribing.
The successful candidate will have excellent written and verbal communication skills, experience of influencing others, and expertise in conducting clinical medication reviews. They will also provide leadership in quality improvement and clinical audit, while supporting delivery of the Quality and Outcomes Framework (QOF) and PCN Network Contract requirements.
