Are you a motivated GP looking to develop your career within an innovative primary care network (PCN) setting?

Beauchamp House Surgery is seeking a dynamic salaried GP to join our multi-disciplinary team as part of a wider PCN, focusing on improving outcomes for our most vulnerable patients through managing frailty and preventing unnecessary hospital admissions.

The position is for 6 sessions per week, looking after patients from our Beauchamp House and Baddow Village Surgeries. You will be required to work on Tuesdays, Thursdays and Fridays each week.

This role offers the opportunity to work across two practices, delivering proactive, holistic care for patients with complex needs, particularly those living with frailty. You will be a key player in a growing ARRS team, collaborating closely with multidisciplinary colleagues including GPs, nurses, pharmacists, paramedics, a digital and transformation manager, social prescribers, general practice assistants and care coordinators.

Main duties of the job

• Undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion;
• Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation;
• Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems;
• Screening patients for disease risk factors and early signs of illness;
• Developing care plans for health in consultation with patients and in line with current practice disease management protocols;
• Providing health education;
• Recording clear consultation notes to agreed standards;
• Collecting data for audit purposes and complying with QOF requirements;
• Delivering enhanced services;
• Compiling and issuing computer-generated acute and repeat prescriptions;
• All general duties and responsibilities associated with a GP working within primary care.

The four surgeries which make up Chelmsford City Health PCN Limited are separate practices that formed a network to benefit from shared resources and to be able to offer healthcare services at scale to its population of 43,000+ patients. Two of the practices are dispensing practices and all four are training practices.

Qualifications
Essential

• Fully qualified GP with GMC registration
• Annual appraisal and revalidation (when appropriate)
• Be on the NHSE medical performers list

Experience
Essential

• Good understanding and experience of chronic disease management, and primary prevention and screening services;
• Ability to take responsibility for your own career development, learning and performance and are responsible for ensuring that all your mandatory training is in date;
• Awareness of and compliance with all relevant practice policies and guidelines, e.g. prescribing, confidentiality, data protection, health and safety;
• Commitment to life-long learning and audit to ensure evidence-based best practice;
• Excellent interpersonal and communication skills in a confidential manner.
