About the Role
Are you an experienced, motivated and compassionate Registered Nurse looking for an opportunity to lead, innovate and make a lasting difference within community and primary care?

Working for Newark Primary Care Network (PCN) means joining a forward-thinking partnership of seven GP practices serving over 79,000 patients across both urban and rural Nottinghamshire. Our teams are committed to delivering high-quality, integrated care that improves outcomes for local people and reduces health inequalities.

We are seeking an experienced Registered Nurse to join our established and highly regarded care home team. This is an exciting opportunity to work across Newark's GP practices and care homes, supporting some of the most vulnerable people in our communities through the delivery of the Enhanced Health in Care Homes (EHCH) framework.

As an experienced Registered Nurse, you will be working alongside a dedicated multidisciplinary team which is currently comprised of: 2x advanced nurse practitioners, 1x clinical pharmacist, 3x nursing associates and 1x care co-ordinator. You will help drive improvements in quality of care, resident experience and clinical outcomes, while supporting people to live healthier, safer and more independent lives.

This is a rewarding role offering a unique blend of clinical leadership, autonomous practice and partnership working.

Key Responsibilities
  • Supporting end-of-life care planning and advance care discussions for the residents in our care homes and completion of ReSPECT forms to ensure their wishes are met at end of life.
  • To lead in the weekly home rounds and have the ability and knowledge to make decisions regards appropriateness of onward referrals and hold others to account for their delivery of actions.
  • A responsibility for ensuring that the residents and care home managers within Newark PCN locality receive assistance in accessing health care interventions by signposting to the most appropriate person to provide care.

Key Requirements
  • Significant post-registration experience within community, primary care, care home or older adult services.
  • Experience of multidisciplinary team working.
  • Strong assessment, care planning and clinical decision-making skills

Job Responsibilities
Key Responsibilities
  • To participate in the overall strategy for development and delivery of the Enhanced Health in Care Homes service across Newark PCN.
  • Will lead in the weekly home rounds and have the ability and knowledge to make decisions regards appropriateness of onward referrals and to hold themselves and others to account for their delivery of actions from the home round.
  • To take a significant role in the holistic assessment and treatment of patients in care homes with complex illnesses. To confidently consult with patients/families/carers to determine the needs of individuals and develop Respect forms to support care.
  • To confidently make autonomous decisions following assessments and ensuring that information is collaboratively communicated back to GP colleagues in a timely and professional manner
  • To make recommendations regarding patient management in accordance with Local and National guidelines
  • Responsibility for consulting and collaborating with other health care professionals and specialists across both primary, secondary, social care and the voluntary care sectors.
  • Demonstrate a good understanding of clinical governance and risk management and apply to work effectively alerting any risks and benefits when developing services and practice in new areas.
  • To facilitate clinical supervision to clinicians who are working within the service and to provide mentorship for clinicians developing advanced skills.
  • Keep abreast of new developments in treatment and technology.
  • Communications across the MDT is a critical element for this role. Information will be shared across providers, and the post holder will support the voice of the residents and care homes as equal members of the MDT.
  • Utilising and interpreting data, to support service improvement for care homes and primary care, as well as understanding the demands on urgent and emergency services to reduce avoidable activity where possible.
  • As a senior clinician there may be a requirement to support local and system wide meetings as appropriate and have a visible presence across the care homes, GP practices and Integrated Care Board work streams which have relevance for care homes in the absence of the clinical lead for the service.
  • The post holder is responsible for ensuring that the residents and care home managers within Newark PCN locality receive assistance in accessing health care interventions by signposting to the most appropriate person to provide care.

  • Demonstrate advanced communication skills in providing advice, empowerment, facilitation, influence, motivation and counselling.
  • Manage complex communication issues that may arise between differing health and social care professionals.
  • Demonstrate an understanding of the need to involve patients and carers in decision making, offering them informed choices, respecting their views.
  • Seek out different styles and methods of communicating to assist longer term needs and aims.
  • The nature of some communication can be unpredictable and can be of an urgent high priority nature: this can therefore be responsible for impacting into time allocated towards other necessary tasks.
  • As a senior clinician manage difficult and challenging conversations, providing clarity and information to seek a mutually agreeable position between parties.

Knowledge, Training and Experience
  • This post demands a range of developed skills from a wide and varied background, experience and a broad yet deep knowledge base underpinned with a proven track record and/or an academically scrutinised knowledge base.
  • Working with care home staff where English is not the first language to promote education and training.
  • Analytical judgement underpinned with specialist knowledge and experience is used to develop evidenced based holistic personal care plan with patients, carers, relatives and health care professionals based on a full assessment of medical, nursing, and social care needs.
  • Provide evidence based written information, ensuring easy access to advice and information for patients and their carers.
  • Acts a role model demonstrating high standards of care and providing clinical leadership to others.
  • Provides specialist education and training to other professionals involved in patient care.
  • Participate in the education, induction programmes and support meetings for other professional staff.
  • Actively participate in other multi-professional meetings, acting as the patient advocate.
