Description

Job summary
  • To provide evidence based clinical assessment and interventions for people with a mild, moderate or severe learning disability who are known to the team.
  • To work with individuals in need of support to help them develop coping strategies that minimise distress.
  • To support staff teams and families to deliver a holistic and person centred approach
  • There is requirement to work in a variety of settings and in collaboration with a number of different agencies.
  • The role requires the post holder to be included in the on-call rota that covers weekends and bank holidays between the hours of 08:00hrs and 18:00hrs
  • To supervise junior colleagues and support students on placement.
  • To be prepared to disseminate assessment outcomes in the most appropriate forum. This may include meetings, staff workshops and training

Main duties of the job
  • Provide assessment of individuals, including risk assessments, using agreed clinical and therapeutic formats to deliver planned interventions as required.
  • Contribute to the development of person-centred support plans in partnership with service users, carers and the multi-disciplinary team.
  • To act in an advisory capacity to individuals, families and support staff, providing evidence based information and training.
  • To be proactive in offering interventions that are the least restrictive option.
  • To determine the appropriateness of referrals and clinical effectiveness.
  • Ensure effective communication and develop close working relationships between service users, families, carers and all relevant agencies, both internal and external.
  • To participate in a supportive multi-disciplinary team.
  • To be able to work autonomously.
  • Ensure adequate written records are kept in line with agreed policies of the Rotherham, Doncaster and South Humber NHS Foundation Trust. To accurately record all visits and contacts, including regular input to the clinical database.

Person Specification
Qualifications
Essential
  • Current UK Professional Registration Registered Nurse Learning Disabilities
  • Evidence of consistent training relating to evidence based care Relevant statutory training
  • Understanding the importance and evidence of commitment to continuing self development and training within a social care or health service delivery setting

Experience
Essential
  • Experience in a variety of care and support settings for people with learning disabilities
  • Experience of care planning for complex health care needs.
  • Experience of report writing
  • Experience of care co ordination

Desirable
  • Experience of coordinating services for people with learning disabilities.
  • Experience of working within a community team.
  • Experience of working with families and carers
  • Experience of supervising staff

Knowledge
Essential
  • Understanding the role and responsibility of the post to all involved organisations and agencies.
  • Clear knowledge and understanding of the statutory policy frameworks relating to services for people with a learning disability. Including mental health act Mental capacity act
  • Ability to analyse and assisting in risk management for clients with forensic histories and risky behaviours
  • Knowledge of current legislation associated with community care.

Desirable
  • Safeguarding risk assessment and management
  • Awareness of communication methods
  • Positive behavioural support training / awareness

Skills
Essential
  • Within allocated caseload ability to prioritise workload and manage time effectively
  • Ability to provide professional support junior staff
  • Ability to maintain safety of yourself and others
  • A team player with good communication skills
  • Implementation of behavioural management strategies and monitoring of.
  • Ability to cope with workload pressure and prioritise workload
