Job Summary
To deliver a health and social care service working as an integral member of the ECHT mental health services, providing a service for people experiencing severe and enduring mental health problems.

The post holder through PROMs, key working and case management will lead and contribute to the provision of a comprehensive range of community services as a Lead professional for a defined caseload.

To be accountable for a clinical caseload delivering appropriate and effective evidence based care to meet the needs of the individual.

To work collaboratively with patients, care home staff and their carers to facilitate and promote rehabilitation, helping patients to achieve and maintain their optimum level of mental health functioning and quality of life in the community care home setting.

Main Duties of the Job
  • To manage an assigned caseload delivering continuous effective evidence based care.
  • To carry out comprehensive patient centered assessments, plan and implement individual strategies of care to meet identified needs in conjunction with the patient, care giver and members of the multi-disciplinary team as appropriate.
  • To evaluate care plans and outcomes regularly and modify as required.
  • To fulfil a care co-ordinator role and ensure that all care given is in line with the requirements of the care management using SystemOne as the primary patient care record.
  • To attend reviews, case conferences, tribunals and other meetings as required.
  • To liaise effectively with all members of the multi-disciplinary team and other agencies involved in the care of the individual thereby maintaining and enhancing close professional working relationships

Person Specification
Qualifications
Essential
  • Degree, Diploma or equivalent in R.M.N, and significant prequalification experience plus additional training/short courses
Desirable
  • Continuing post qualification Mental health training
  • Experience of working with People with mental health problems
  • V300 independent supplementary nurse prescribing or willing to work towards it

Experience
Essential
  • Appropriate experience either pre or post qualification in the field of mental health
  • Crisis working Management of complex needs
Desirable
  • Work in multi-disciplinary setting
  • Local Government experience Mental Health experience Care Programme Approach Care co-ordination.
  • Experience of collaborating with older people with mental health problems

Knowledge
Essential
  • Understanding of Equal Opportunities' Policy. Knowledge of the National Health Service and related organisations.
  • Knowledge and understanding of current mental health issues.
  • Knowledge of the Mental Health Act and Mental Capacity Act and Care Act including other relevant legislation.
  • Understanding of the Care Programme - understanding of dialogue
  • Approach and ability to fulfil a care co-ordinator role.
  • Assessment and organisational skills.
  • Ability to prioritise work.
  • Range of intervention skills.
  • Problem solving skills.
  • Evidence of co-ordinating /planning interagency approaches to mental health care.
  • Ability to work as part of a team and also without direct supervision.
  • Excellent communication skills in individual and group settings High standards of literacy and IT skills.
Desirable
  • Experience in mental health work.
  • Experience in delivering psychosocial interventions and participating in assessments of older people.
