This person will also need to be committed to a high quality standard for patient care.
The post holder will be based at Springfield university hospital in Tooting. There is an expectation that the post holder is able to work shifts Patterns. The candidate will be part of the multi-disciplinary team, working mostly independently. You must be able to have good communication skills, be organised and possess good time management. A car driver/owner is essential. We are committed to continuous professional development and you will receive regular supervisions to adapt to the needs of the service and to work towards PADR indicators. Main duties of the jobWandsworth Crisis Resolution and Home Treatment Team consist of a multidisciplinary, community based mental health team that aims to provide a safe and effective home-based assessment and treatment service as an alternative to in-patient admission. We can facilitate early discharge from the ward in the least disruptive and restrictive environment. We work in conjunction with, as well as integrate with other local service elements. The workload is team-based and our team members do not carry a specific caseload themselves, instead they share one as a collective unit.
The response will include assessments at MHA's and home visits. Staff will undertake rapid assessments and then either refer or signpost to other services. Out of hours activity will be coordinated through the Mental Health support Line and responded accordingly. Wandsworth Crisis Resolution and Home Treatment Team offer an alternate to acute psychiatric admission, and the team acts as 'Gatekeepers' to the acute ward and the team works closely with the Trusts in patient's wards to help action this effectively. The Wandsworth Crisis Resolution and Home Treatment Team helps to facilitate early discharge and provides discharge support to clients in Wandsworth and facilitating the forwarding clients on to alternative services.

Job responsibilitiesThe post holder will be a member of the Wandsworth Crisis and Home Treatment Team. The post holder will also have responsibility of working some weekend and night shifts which may entail undertaking assessments or offering support across geographical borders within South WestLondon.
The main target group are people with severe mental health problems experiencing mental health crisis.
The Wandsworth Crisis & Home Treatment Team (CHTT) is a multidisciplinary, community based mental health team that aims to provide a safe and effective home based assessment and treatment service as an alternative to in-patient admission, and for early discharge from the ward in the least disruptive and restrictive environment. It works in conjunction with, and is integrated with, other local service elements. The workload is team-based and team members do not carrya specific caseload. Care by the team is for days or a maximum of 6 weeks but not longer.
The Team is available to provide care and intervention 24 hours per day, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of the CHTT.
The SWLSTG Mental Health Trust is required to gate keep all admissions to hospital.
The key features of the service are that it is accessible, mobile, responds quickly and acts as the gatekeeper to in-patient services and assessments for psychiatric presentations. It offers care within a bio/psycho/social model with emphasis on symptom management, within a Recoveryand Wellbeing Model and encouraging the service user (and families) to cope and understand their crisis.

- Evidence of continuing professional and personal development Full and current drivers licence and car owner (or willing to take part in car lease scheme run by the Trust)
Desirable- Mentorship ,ENB 998 or equivalent
- Diploma in cognitive behavioural Therapy/psychodynamic counselling
ExperienceEssential- Demonstrable experience of working with people who experience serious ongoing mental health problems in a community or outpatient setting post qualification
- Experience of working in a multidisciplinary team
- Experience of assessing and managing crisis with those with a severe mental illness
- Experience of working as a clinical supervisor
Desirable- Experience of managing and supervising staff
- Experience of contributing to policy and procedure development and implementation
