Job SummaryAn exciting opportunity has arisen for 1 WTE Band 6 Community Specialist Practitioners (Health Visiting / School Nursing) to join the already established and dedicated Children In Care (CIC) Team in Doncaster. The successful applicants will be part of a multi-disciplinary team providing holistic health services to CIC & Care Leavers aged 0-19 years.
Main Duties of the Job- Undertake review health assessments for Children In Care and Care Leavers within their defined caseload
- Contribute to the Healthy Child Programme as required
- To improve the quality and co-ordination of the health care of Children In Care and Care Leavers.
- Contribute to assessing, monitoring and evaluating health needs and improving health outcomes for Children In Care and Care Leavers
- Undertake evidence-based packages of care based on identification of health needs
- To work in partnership with the Named Nurse and Named GPs for CIC and the dedicated CIC Nursing Team
