Job Summary
An exciting opportunity has arisen for 1 WTE Band 6 Community Specialist Practitioners (Health Visiting / School Nursing) to join the already established and dedicated Children In Care (CIC) Team in Doncaster. The successful applicants will be part of a multi-disciplinary team providing holistic health services to CIC & Care Leavers aged 0-19 years.

Main Duties of the Job
  • Undertake review health assessments for Children In Care and Care Leavers within their defined caseload
  • Contribute to the Healthy Child Programme as required
  • To improve the quality and co-ordination of the health care of Children In Care and Care Leavers.
  • Contribute to assessing, monitoring and evaluating health needs and improving health outcomes for Children In Care and Care Leavers
  • Undertake evidence-based packages of care based on identification of health needs
  • To work in partnership with the Named Nurse and Named GPs for CIC and the dedicated CIC Nursing Team

Qualifications
Essential
  • NMC registered qualification
  • Community Specialist Practitioner (Health Visiting /School Nursing)
  • Evidence of continued professional development in areas relevant to Children, Young People and Families

Experience
Essential
  • Working with Children, Young People and Families
  • Working with Children, Young People and Families with safeguarding concerns and complex needs
  • Working with Children In Care, Young People and their Carers
  • Working within the Community setting

Desirable
  • Experience of working in close partnership with other agencies involved in the care of CIC; including social services, foster carers, residential care services, CAMHS, Paediatrics, Sexual Health services and Substance Misuse services.
  • Demonstrates understanding of Looked After Children's Policy and Guidance and implications for service delivery.

Knowledge
Essential
  • Well organised and skilled in good time management.
  • Evidence of excellent verbal and written communication skills.
