We are looking for a motivated and compassionate Paediatric Diabetes Specialist Nurse (PDSN) to join our progressive and supportive team!

Shropshire Community Health NHS Trust has an exciting opportunity for an experienced Paediatric Diabetes Specialist Nurse to join our Team as Paediatric Transition Diabetes Specialist Nurse.

The Paediatric Diabetes Specialist Nursing Team works alongside Shropshire and Telford Hospitals NHS Trust to deliver high quality, safe and effective care for children / young people, their families and carers. It offers multi-disciplinary clinics with Consultant, Nurse, Dietician and Psychologist across Shropshire, Telford & Wrekin and Powys.

The Paediatric Diabetes Specialist Nursing Team is supportive of flexible working arrangements and newly recruited staff are offered a comprehensive induction package.

Main duties of the job
To assess, plan and implement and evaluate evidence-based nursing care for children and young people.

To provide effective nursing expertise, specialist advice and support to the nursing team and the wider MDT to ensure high quality care is delivered across Shropshire, Telford and Wrekin.

To work closely with the wider MDT to provide support and direction in the education and training of patients and their families to deliver the highest standards of clinical care.

Promote a "can-do" attitude and determination to develop and sustain dynamic and responsive multidisciplinary/ multi agency community services within the county which delivers best practice.

Manage a complex specialised caseload providing high quality clinical education, advice, information and support to CYP and their carers/ families on all aspects of their condition and its effect on their lives

Training and Qualifications
Essential
  • Registered Children's Nurse/RN (Child)
  • Highly developed specialist knowledge achieved through first level degree or equivalent portfolio of evidence
  • Evidence of short courses at master's level or equivalent experience
  • Recognised Qualification in field of practice
  • Teaching Certificate or equivalent experience
  • Independent Nurse Prescriber
  • Counselling qualification

Experience
Essential
  • Extensive post registration experience in field of practice
  • Experience of provision of training/education to range of staff and service users
  • Assessing and prescribing care and treatment plans
  • Participation in service development and the implementation of change

Knowledge and Skills
Essential
  • Ability to manage complex caseload of high-risk patients
  • Ability to communicate complex and distressing information to patients and their carers in a sensitive and supportive way at a level they can understand
  • Ability to manage complex caseload of high-risk patients
  • Ability to take initiative, work autonomously, make decisions and prioritise workload
  • Excellent interpersonal and leadership skills

Essential
  • Ability to meet travel requirements of the post which will include travel across Shropshire and Telford and Wrekin localities and wider to cover the needs of the post
