About the Role
As a Nursing Associate within the Children's Community Nursing Team, you will play a key role in delivering person-centred care and supporting children and young people to remain at home whenever possible.Working under the direction of registered nurses, you will contribute to assessments, care planning and the delivery of clinical interventions while promoting the health, wellbeing and independence of children and their families.The role offers a varied and rewarding workload, including supporting children with complex health needs, undertaking delegated clinical procedures and working closely with families, schools and partner agencies.

Main duties of the job
The Nursing Associate has a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person-centred care and support for people of all ages and in a variety of settings. The Nursing Associate works independently under the leadership of registered nurses _,_ working within the sphere of nursing and care and within all aspects of the nursing process.

Nursing Associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.

The following criteria identify the core components of the roles, responsibilities and accountabilities of Nursing Associates. They identify what Nursing Associates can contribute to the health and wellbeing of patients and service users and apply across all health and care settings. Integral to all of these is the ability to communicate effectively, with sensitivity and compassion, and to manage relationships with people, making reasonable adjustments where necessary.

Nursing Associates will be required to advocate for Children and Young People (CYP) referred to Children's Community Nursing Team (CCNT). The post holder will be required to support at times with Clinical Education, Palliative Care, Generic CCN work and to support with caseload management of Children known to Continuing Care

Job responsibilities

Promoting Health and Preventing Ill Health

• Support CYP to improve and maintain their mental, physical, behavioural health and wellbeing where possible dependent on the trajectory of the CYPs condition.
• Active involvement in the prevention of and protection against disease and ill health
• Engage in public health, community development, and in the reduction of health inequalities
• Support health and social care colleagues with completing assessment of CYP.

Provide and Monitor Care

• Provide compassionate, safe and effective care and support to CYP in a range of care settings.
• Undertake training and complete competencies for specific nursing skills in order to facilitate care provision and assessment.
• Monitor the condition and health needs of CYP within their care on a continual basis in partnership with people, families, and carers
• Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment
• Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care
• Recognise and report any situations, behaviours or errors that could result in poor care outcomes
• The post holder will be required to use a computer, either stand alone or as part of a networked system and will be responsible for the quality of the information. The amount of time spent on this type of work will depend on the job.
• All staff has a duty to ensure that all relevant manual and computerised data is collected and entered in an accurate and timely manner, ensuring all activity undertaken is accounted for.

• Play an active role as members of interdisciplinary teams

• Improve the quality of care by contributing to the continuous monitoring of CYP and families experience of care.
• Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of CYP first.
• To participate in effective and comprehensive audits and measure against benchmarked targets set within the framework.
• Be aware of and maintain the medical equipment database to ensure all equipment is serviced as per SWFT policy.
• To contribute to policy reviews.

Contributing to Integrated Care

• Contribute to the provision of care for CYP, including those with complex needs
• Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a CYP and their family
