We have a very exciting new opportunity to recruit into a Band 6 role at the North Kirklees EIP team. We are looking for a self motivated, enthusiastic individual who has a desire to work to improve life chances of people who may be experiencing their first episode in psychosis. This can take a lot of patience, requiring keen skills in creative engagement. Part of the role will involve health promotion; improving the physical heath of this group of service users as well as promoting the role of the insight team to capture appropriate referrals

The team covers the North Kirklees locality, working with people aged 14 to 65 and stretches across adult and childrens services. The team forms part of the community service line and Enhanced Pathway within the North Kirklees BDU. We will consider applications from an experienced Band 5 or Band 6 who wish to transition into the role with a fully supported integration and coaching process.

Main duties of the job

The post holder will be responsible for coordinating the care of approx. 15 service users who have experienced first episode of psychosis or are at ultra-high risk of developing it.

Providing practical, social, emotional and vocational support for individuals aged 14-65 following a diagnosis of a psychotic illness. To enable service users to exercise informed choices about their lives where possible and to enhance their social skills and ability to participate in the community. Provide a recovery focused service to individuals to ensure they are aware of their rights and responsibilities within the project and society. Support individuals through a tailored support programme with a view to move on to community engagement and vocational opportunities. Supporting individuals through a range of activities, on an off-site and peripatetic basis. Facilitating the use of local community services. Identifying and meeting the needs of individuals to enable them to maintain their health and wellbeing through engaging individuals in social inclusion groups as well as the cardio-metabolic assessment clinic. Providing information, advice, and guidance as appropriate. Assisting individuals in identifying areas of support and in line with their Care & treatment plan.

Job responsibilities

JOB SUMMARY

Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as a member of an Early Intervention in Psychosis team.

Key Result Areas:

• Management of own caseload in line with C.P.A. procedures and risk management protocols.
• Provision of leadership to the Early Intervention in Psychosis team in respect of psycho-social interventions to people and their carers in line with national and local practice standards e.g. NICE
• Supervision and support of other team members.
• Participation in service development and clinical governance structures.

Professional:

• Assesses needs and identifies problems relevant to the care of people referred.
• Devises a plan of care in partnership with the person and carers.
• Implements the planned programme of care to ensure a high standard is achieved.
• Reviews the effectiveness of the care provided and where appropriate initiates any action.
• Advises and supports the person and carers to promote health and well-being and to prevent illness.
• Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
• Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.
• Functions as a member of a multi-disciplinary team.
• Management of enquiries and direct referrals of people in accordance with Early Intervention in Psychosis team operational policy and participates in the team duty rota.
• Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
• Responsible for acting up in the absence of the team leader

Administrative:

• Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
• Participates in the formulation of policies at unit level.
• Maintains accurate mileage records.
• Manages caseload with due economy of cost and time.
• Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).
• Responsible for the ordering of stock with due economy.
• Produces monthly statistics in line with trust directives.
• Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.

Essential

• Can demonstrate skills in assessing and managing people experiencing complex mental health problems.
• Experience of supporting and supervising colleagues/students/trainees.
• Experience of using IT systems to support, record and monitor clinical activity.
• Can demonstrate skills in assessing and managing risk in a variety of settings.
• Proven ability in assessing the needs and strengths of people and their carers Experience of delivering a range of therapeutic interventions.
