Would you like to make a meaningful difference by helping people receive high-quality end-of-life care in the comfort of their own home? We are looking for a compassionate and dependable Home Nursing Assistant to join our St Catherines at Home team.

You will work in patients homes, under the direction of a Registered Nurse, providing personal care, practical support and reassurance to patients and their families. Our Care Directorate provides services 24 hours a day, seven days a week, in both community and inpatient unit settings so flexibility is essential.

Main duties of the job

• Provide dignified personal care and emotional support in line with individual care plans.
• Support patients to remain at home where possible and appropriate.
• Recognise and report changes in a patients condition or circumstances.
• Use safe moving and handling techniques and respond appropriately in urgent situations.
• Maintain accurate written and electronic care records.
• Communicate sensitively with patients, families and healthcare professionals.

Job responsibilities
Care delivery

• Provide personal care to patients approaching the End of Life and support for their carers, enabling the patient to stay at home where possible and desirable.
• To provide care to the patient under the supervision of the Team leader or the District Nurse and according to the nursing care plan, this will include practical nursing care and emotional support.
• To continually asses/ reassess the moving and handling needs of the patients to ensure patient and staff safety
• To know how and who to contact in an emergency when an urgent situation arises
• Move and handle patients, safely using up to date moving and handling techniques.

Communication

• To communicate closely with the District Nurses and/ or the St Catherines at Home team leader and report any change in the patients condition or circumstances
• Accurately complete records of care electronically and written (including electronic and district Nursing notes)
• Communicate with patients and families in a sensitive and empathetic manner
• Contribute to the training needs of staff, volunteers and students on placement.

Experience
Essential

• Ability to contribute to written reports
• Experience within a care environment
• Good interpersonal skills
• Good verbal and non-verbal communication skills

Desirable

• Previous experience in working in a community setting
• Previous experience of caring for patients approaching the End of Life

Essential

• Resilience to work effectively in a patients home.
• Access to a car on a daily basis and full driving licence essential

Qualifications
Essential

• Driving licence
• Demonstrate required level of numeracy and literacy skills (either during the selection process or GCSE grade C or equivalent)
• Completion of care certificate pr willingness to complete

Desirable

• GCSEs in English and Maths
• RQF Level 2 in Health and Social Care or equivalent to NVQ/ QCF/ CACHE level 2 in Health and Social Care
