We are seeking an enthusiastic and motivated Clinical Support Worker to join our Frailty, Later Life and Dementia Service. The service supports older adults experiencing functional mental health difficulties, memory problems, dementia, and behaviours that challenge, while working closely with families and carers to provide person-centred care.

As a valued member of the team, you will work alongside qualified professionals to support, supervise and educate clients, helping them maintain their independence and wellbeing within their local community. You will work collaboratively with clients, carers, colleagues and partner agencies to assess needs, identify goals and deliver high-quality, person-centred support that promotes dignity, choice and positive outcomes.

Main duties of the job
The post holder will support the assessment, planning and delivery of person-centred care under the guidance of a supervising clinician, ensuring care is provided in line with clinical governance standards. They will undertake and accurately record physical health checks, including supporting individuals during clozapine titration and promoting their overall physical wellbeing. The role also includes leading on carer support within the team, ensuring carers are identified, involved and provided with appropriate information and support while individuals are receiving care from the service.

Qualifications
Essential

• Foundation Degree in Mental Health or related subject.
• OR Level 3 Qualification in health-related area and two years' experience in a care role.
• Significant experience in role delivering care in a mental health setting.
