This role is ideal for a nurse with general practice primary care experience who is looking to develop their skills within a well-established and progressive GP practice.
Main Duties of the Job- Deliver high-quality nursing care within a primary care setting
- Undertake routine nursing duties including immunisations, cervical cytology, and wound care
- Support patients with long-term condition management (e.g. diabetes, asthma, COPD)
- Carry out health promotion and screening activities
- Work collaboratively with the wider multidisciplinary team
- Maintain accurate clinical records in line with practice policies

Our multidisciplinary team includes:
- 4 GPs
- 1 Advanced Nurse Practitioner (ANP)
- 2 Clinical Pharmacists
- Nursing team
- Additional PCN staff
We work closely as a team to deliver safe, effective, and patient-centred care.
Job ResponsibilitiesTo deliver high-quality evidence-based nursing care to patients registered with the practice. Supporting the clinical team to provide safe, effective, and responsive primary care services. The Practice Nurse will work autonomously within their scope of practice while contributing to multidisciplinary team working and population health management.
Clinical ResponsibilitiesProvide general practice nursing services, including:
- Chronic disease management - e.g. asthma, COPD, diabetes, hypertension, CHD
- Cervical cytology - As trained and competent
- Childhood and adult immunisations
- Travel health advice and vaccinations
- Wound care, dressings and leg ulcer management
- Ear care and basic health checks
- Perform health promotion and preventative care, including lifestyle advice - smoking cessation, weight management, alcohol awareness
- Undertake clinical assessments, planning, implementation and evaluation of care
- Recognise and respond to urgent clinical situations and escalate appropriately
- Order, interpret and act upon tests within agreed practice protocols
- Maintain accurate, contemporaneous clinical records using the practice clinical system
Safeguarding and Patient Safety- Actively safeguard children and vulnerable adults in line with local and national guidance
- Maintain awareness of safeguarding policies and participate in training as required
- Identify and report significant events, incidents, near misses and concerns
- Contribute to infection prevention and control measures and audits
Medication Management- Administer medicines and vaccines safely in line with PGDs, PSDs and practice policy
- Support medicines optimisation and monitoring as delegated
- Identify and report adverse drug reactions
Professional Practice- Practice in accordance with the NMC Code, maintaining professional registration
- Work within recognised clinical guidelines and practice policies
- Maintain competence through continuing professional development - CPD
- Participate in clinical supervision, appraisal and revalidation processes
- Teach or support patients, carers, students and new staff where appropriate
Teamworking and Communication- Work collaboratively with GPs, HCAs, pharmacists, paramedics, PCN staff and community services
- Communicate effectively with patients, families and colleagues
- Contribute to MDT meetings and service development where appropriate
- Support the practice in developing nurse led clinics and pathways
Quality and Governance- Participate in clinical audits, QOF delivery and quality improvement activity
- Support compliance with CQC standards and practice governance requirements
- Contribute to health inequalities initiatives and population health work
- Use IT effectively for clinical documentation and recall systems
Person SpecificationEssential- Registered Nurse (Adult) with current NMC registration
- Must have general practice experience
- Competent in core practice nursing skills
- Good communication and interpersonal skills
- Ability to work independently and as part of a team
- IT literate - SystmOne experience desirable
- General Practice Nursing qualification
- Chronic disease management qualifications
- Cervical cytology qualification
- Travel health qualification
- Experience of QOF and audit work
