Job summary
The postholder will work as an autonomous practitioner, with extended scope of practice, providing specialist assessment and management of children and young people with diabetes (CYPD) at diagnosis and throughout their journey. They will use their excellent knowledge of current evidence-based practice as well as advanced clinical reasoning skills.

Main duties of the job
To work as an autonomous practitioner providing advanced CYPD dietetic assessment and management to a specific specialised clinical caseload.

To develop comprehensive management and treatment plans, utilising extensive diabetes knowledge to formulate a specialised empowering and progressive programme of care, whilst always considering the impact of cultural and demographic factors.

To work with the MDT as a vocal member of the team contributing essential information and in-depth clinical knowledge to the service.

To contribute specialist knowledge to the CYPD MDT. Integral to the planning, implementation and monitoring of a highly specialist, evidenced based, clinically effective service, thus achieving the Best Practice Tariff criteria.

To make significant contributions to policy development within paediatric diabetes.

To act as an expert resource on paediatric diabetes including, nutrition, medication, and diabetes technologies and to provide ongoing education and training to all existing and new members to the CYPD MDT, paediatric ward and A&E staff including doctors and nurses, as well as dietetic, nursing and medical students within the trust

To be actively involved in the Trust's clinical governance program by taking lead responsibility for ensuring continuing professional development, audit, research and service development activities within this specialist area.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust's services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.

Education and Qualifications
Essential
  • BSc Degree in Nutrition and Dietetics or Diploma or equivalent
  • Evidence of current Health and Care Professions Council (HCPC) registration (State Registered Dietitian)
  • Post graduate training or development in clinical skills up to MSc level with a focus in Children's Diabetes
  • Supplementary Non-Medical Prescribing qualification attained or willingness to undertake qualification

Desirable
  • Member of the British Dietetic Association or covered by professional indemnity insurance
  • Specialist training in the area of paediatrics dietetics
  • Membership of BDA special interest group

Experience
Essential
  • Substantial postgraduate experience in children's diabetes and or T1 diabetes and general dietetics (acute/community) including diabetes and paediatrics
  • Advanced knowledge and experience of a wide range of approaches to the management of patients with diabetes including HCL therapy and group work
  • Experience in managing patients with multiple comorbidities in group and individual settings
  • Experience of supervision of junior staff, recruitment, and objective setting/ personal development plans
  • Experience of conducting (patient & staff) teaching groups within an MDT setting
  • Evidence of service development and contribution to change within the workplace
  • Active involvement in clinical audit, research and policy/guideline implementation

Desirable
  • Experience interpreting HCL data from systems such as Glooko
  • Experience using the QI model for service improvement

Skills, knowledge and abilities
Essential
  • Excellent working knowledge of a range of clinical conditions and current best practice for dietetic management in particularly diabetes, paediatrics and coeliac disease
  • Knowledge of recent developments in diabetes treatments and guidelines
  • Skills in critical analysis and the application of research to practice
  • Ability to support and facilitate change
  • Excellent communication skills, both written and verbal
  • Ability to demonstrate excellent organisational and advanced time management skills to complete allocated projects on time
  • Ability to work within and apply Trust policies, including clinical governance, data protection, equal opportunities, and health and safety, to meet the differing needs of the patient
  • Ability to deal with distressed clients and relatives and to manage stressful situations by remaining calm, objective and professional at all times
  • Able to carry out concurrent activities and work in an unpredictable working pattern
  • Ability to lead in service developments, liaising closely with the MDT in order to promote clinical excellence
  • Proficiency in the use of IT applications to support efficient work activity e.g. Microsoft packages, nutritional assessment packages, statistical packages and electronic records
  • Effective training and presentation skills using a variety of teaching methods for individuals and groups
  • Excellent leadership skills
  • A commitment to professional development for self and others
  • Demonstrate an understanding of NHS reforms on policies and their impact on service
  • Ability to recognise and act on any safeguarding concerns
  • Advanced clinical reasoning, critical appraisal and analytical skills
  • Ability to work as an autonomous practitioner/ independently using own initiative and with individuals and groups and as part of the MDT

  • Full UK driving licence and willingness to drive to offsite clinics and home visits
