Job summary
To provide advances knowledge and skills in diabetes nursing care to patients and their carers in the community setting. To be an expert resource support and educated Health Care Professionals across divisional and organisational boundaries. Support and design community education programmes. Manage the introduction of diabetes treatment guidelines, protocols and complete audit and research projects.

Main duties of the job
To work as an expert independent practitioner in Plymouth city community area, supporting GP practices and Care Homes, receiving referrals from GPs, Practice Nurses, LTC Matrons, District Nurses, Consultants and other sources. Hold monthly locality nurse led clinics in GP surgeries and patient reviews in Care Homes. Support Consultant Diabetologists in their virtual and locality clinics.

To lead on the development and implementation of comprehensive clinical care pathways and education programmes in line with current best evidence, policies and guidelines. Support and supervise junior team members in the provision and evaluation of type 2 diabetes group education.

Job responsibilities
  • Works across Care Home and Independent Living settings, providing education, expert clinical advice and plans, reviews and adapts individual plans of care.
  • Works autonomously as an independent practitioner to manage his/her own work-plan and time whilst working as part of a multidisciplinary team.
  • Utilises advanced skills of assessment, interprets complex psychological, physiological, emotional, spiritual and cultural needs.
  • Enables patient choice and involvement and initiates appropriate action.
  • Co-ordinates patient care pathway and liases across organisational boundaries.
  • Plans specialised programmes of care and provides highly specialised advice to patients, relatives and carers.
  • Participates in patient case conferences in relation to complex management as an expert resource in diabetes care.
  • Ensures patients and their families are given appropriate oral and written information regarding planned care and the disease process.
  • Maintains accurate patient records and ensures all relevant information is documented in the patients medical and nursing records.
  • Ensure each patient is placed in the correct treatment pathway at the appropriate time and necessary investigations and treatments are organised.

Person Specification
Knowledge
Essential
  • Demonstrable experience DSN with highly specialised skills.
  • Demonstrated experience at a senior nurse level
  • Participated in previous service development and audit
  • Able to manage complex relationships
  • Presentation skills
  • Insight and knowledge of recent NHS legislation and recommendations

Desirable
  • Demonstrated experience of managing teams.
  • Change management skills and involvement in previous projects.
  • Demonstrable Experienced nurse prescriber

Qualifications
Essential
  • RN Diploma qualification in nursing studies.
  • Post registration specialist qualification where appropriate.
  • Degree module in specialist area, or working towards this or be prepared to study at this level
  • Teaching qualification. ENB 998 or City & Guilds 730 and demonstrable experience of teaching and assessing.
  • Continuous personal and professional development
  • Nurse prescriber or willingness to complete the training.
  • Valid UK driving licence and use of own vehicle

Desirable
  • MSc/degree qualification
  • Communication/counselling skills training
  • Leadership / management training

Aptitude & Abilities
Essential
  • Reliable, highly motivated and dedicated individual, who is keen to advance the community diabetes service.
  • Advanced communication skills.
  • Leadership: facilitation, negotiation skills
  • Proven Teaching ability

Desirable
  • Presentation at regional/ national conferences
